Recommendation:
AUTHORIZE relief of cash shortage in the amount of$271.48 as recommended by the Auditor-
Controller.
FINANCIAL- IMPACT:
Cash shortage in the amount of$271.48.
REASONS FOR RECOMMENDATION / BACKGROUND:
In accordance with the provisions of Administrative Bulletin 207.6, the Health Services
Department requested relief of a cash shortage in the amount of $271.48. It has been
determined that the shortage was not due to negligence on the part of the department. The
Auditor-Controller recommends and the County Administrator's Office concurs that the Board of
Supervisors grant relief of shortage to the Health Services Department for these funds.

Subject: Relief of Cash Shortage
Please consider this memo a request for relief of a cash shortd�e'of$271.48 at the Pittsburg Health
Center,
Sometime between 5;00 PM Saturday 17 June 2000 and 10.45 AM Monday 19 June 2000 a theft
occurred at the Pittsburg Health Center. The missing money represents $98,56 in Petty Cash Change
Fund and$172.92 in Patient Receipts. The Pittsburg Police Department was contacted and did respond
to complete an incident report, Pittsburg Pb has closed the case due to"No suspects or workable leads
at this time."
Attached you will find a copy of the Pittsburg PD report and Tanya Gutierrez, Ambulatory Care
Registration Supervisor's memo to George Washnak with the known details of the theft. Tanya's memo
also details the steps she has taken to prevent this type of occurrence in the future. I have also
suggested to Jose Saavedra, Ambulatory Care Registration Manger that during the planning phase of
the Los Medanos project he look into putting a safe with dual access control in the Registration area,

SUBJECT: Health Services Department Report of Shortage and Request for Relief of Accountability
We have reviewed the attached report of a theft at the Pittsburg Health Center, which resulted in a loss
of$271.48 undeposited collections and revolving funds, and the re4u6st for discharge from
accountability for these funds.
The Health Seruices has since improved cash handling procedures at the Pittsburg Health Center to
mitigate control weaknesses that precipitated this loss. In our opinion, implementation of the security
measures will deter reoccurrence of the events that resulted in this loss.
In accordance with the provisions of Administrative Bulletin 207.6, since amount of loss exceeds $250
the matter should be presented to the Board of Supervisors for action. We recommend that the Board
of Supervisors grant relief of shortage and accountability for these funds.

Programs participation in the February 3, 2001,"Office of Emergency Services Fair".

participates in community events as part of their endorsement to their educational process.
Participation includes the offering of educational information in regards to the Community Warning
System,hazardous materials,household hazardous waste, and the proper handling thereof.
