WEIR, Clerk-Recorder WHEN RECORDED, RETURN DOC— 2000-r0256040-00 TO CLERK, BOARD OF Wednesdoy, NOV 15, 2000 13:19:40 SUP tiYISORs7 FRB $0.00 Ttl Pd $0.00 Nbr-0000167816 1 re/RS/1-2 A/D File: 250-9963/C.1.1 BOARD OF SUPERVISORS, CONTRA COSTA COUNTY, CALIFORNIA In the Matter of Accepting and Giving RESOLUTION OF ACCEPTANCE Notice of Completion of Contract with and NOTICE OF COMPLETION R-E Corporation, dba: Reliance Enterprises (C.C. §30$6, 309T/527 Budget Line Item No. 6971-4614 RESOLUTION NO. 20 The Board of Supervisors RESOLVES THAT: The County of Contra Costa on March 21, 2000, contracted with R-E Corporation, dba: Reliance Enterprises, for a Stand Alone Boiler for the Family Practice Center, Contra Costa County Regional Medical Center, 2500 Alhambra Avenue, Martinez, Budget Line Item No. 6971-4614, Authorization No. 0927-WH614B, with St. Paul Fire and Marine Insurance Company as surety, for work to be performed on the grounds of the County; and The Director of General Services reports that said work has been inspected and complies with the approved plans and specifications, and recommends its acceptance as complete as of November 14, 2000; and recommends the withholding of$2,500.00 pursuant to a letter dated October 30, 2000; Therefore, said work is accepted as recommended above, and the Clerk shall file with the County Recorder a copy of this Resolution and Notice as a Notice of Completion for said contract; and the Director of General Services is directed to withhold monies otherwise due pursuant to the letter dated October 30, 2000. Time extension to the date of acceptance is granted as the work was delayed due to unforeseeable causes beyond the control and7 without the fault or negligence of the Contractor. County Recorder RL:tb H:\1999\2509963\9L06353b.doc Page 2 of 2 File: 345-9802lC.1.1 > THE BOARD OF SUPERVISORS CONTRA COSTA COUNTY, CALIFORNIA Adopted this Order of November 14, 2000,by the following vote: AYES: SUPERVISORS GIOIA, UILKEMA, OESAULNIER, CANCIAMILLA AND GERBER NOES: NONE ABSENT: NONE RESOLUTION NO. 2QO01526 SUBJECT: Approving Plans and Specifications for Remodeling Diagnostic Imaging Department at Los Medanos Health Center, 2311 Loveridge Road,Pittsburg for Health Services Department (CP#98-11) Budget Line Item No. 6973-4604 Authorization No. 0927-VY H604B WHEREAS plans and specifications for Remodeling Diagnostic Imaging Department at Los Medanos Health Center, 2311 Loveridge Road, Pittsburg for Health Services Department have been filed with the Board this day by the Director of General Services; and WHEREAS plans and specifications were prepared by Fong& Chan Architects; and WHEREAS the cost estimate for the initial construction contract is $105,000.00; and WHEREAS the Board has previously found on February 18, 1998 that the project is a California Environmental Quality Act(CEQA) Class 1 a Categorical Exemption, and the Director of Community Development has filed a Notice of Exemption with the County Clerk; and DIRECTS the Director of General Services to arrange for payment of a $25.00 handling fee to Community Development for administration processing costs; WHEREAS the Board has obtained the general prevailing rates of wages, which shall be the minimum rates paid on this project; and IT IS BY THE BOARD RESOLVED that said plans and specifications are hereby APPROVED. Bids for this work will be received on December 21, 2000 at 2:00 p.m., and the Clerk of this Board is directed to publish a Notice to Contractors in accordance with Section 22037 of the Public Contract Code, inviting bids for said work, said Notice to be published in CONTRA COSTA TIMES . The Director of General Services is directed to mail notices to the construction trade journals specified in Section 22036 of the Public Contract Code at least 30 days before the date of opening the bids. IT IS FURTHER RESOLVED that the Director of General Services, or his designee, is AUTHORIZED to issue bid Addenda, as needed, for clarification of the contract bid documents, provided the involved changes do not significantly increase the cost estimate for the initial construction contract.
