SUBJECT: Approval of Contract #26-398 with the U.S. Department of
Veterans Affairs
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Frank
Puglisi, Jr. ) , to execute on behalf of the County, Contract #26-398 (V261S-
0426) with the U.S . Department of Veterans Affairs Northern California
Health Care System (VANCHCS) , in an amount not to exceed $80, 142 for the
provision of Dermatology services, for the period from November 1, 2000
through September 30, 2002 . In accordance with the term of the agreement,
County agrees to indemnify and hold harmless Contractor from claims, losses,
damages, liabilities, costs, expenses or obligations arising out of, or
resulting from CCRMC' s wrongful or negligent conduct in the performance of
this Contract .
FINANCIAL IMPACT:
The Agreement is funded in the Health Services Department ' s Enterprise I
Budget . Under the term of the agreement, VANCHCS is paid a fee for service
in accordance with the fee schedule which is incorporated in the Contract .
The services provided for the County' s patients under this Contract are
billable to patients and third party payors .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
For many years the County and U.S . Department of Veterans Affairs has
maintained a mutual sharing agreement which has made available to the County
specialized medical services not otherwise available due to lack of
resources, equipment, and personnel . These services included specialized
laboratory testing, radiology services, nuclear medicine studies, CT scans,
MRI ' s, dermatology, gastroenterology, urology, audiology and speech, and
ophthalmology services . The County provided Emergency Room treatment and
inpatient care, including certain ancillary services for VA-referred
patients .
Approval of Contract #26-398, will allow Contractor to provide specialized
Dermatology services to County' s patients referred for treatment by Contra
Costa Regional Medical Center physicians through September 30, 2002 .
The Contract documents have always been prepared by the Veterans
Administration.

Crestwood Behavioral Health, Inc.

Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Contract
Amendment Agreement #24-933--10 with Crestwood Behavioral Health,
Inc . , effective June 1, 2000, to amend Contract #24-933-6 to
increase the payment limit by $317, 944 from $4, 258, 681 to a new
total Contract Payment Limit of $4, 576, 625 and to increase the
Automatic Extension Payment Limit by $561, 257 from $2, 129, 341 to a
new total of $2, 690, 598 .
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment .
BACKGROUND/REASON(S) FOR RECOI,ENDATION(S) :
On September 28, 1999, the Board of Supervisors approved Novation
Contract #24-933-6 (as amended by Contract Amendment Agreements
#24-933-8, and #24-933-9) with Crestwood Behavioral Health, Inc. ,
for the provision of admission and intensive day treatment of
County-referred mentally ill persons who are in need of sub-acute
skilled nursing care in an IMD, for the period from July 1, 1999
through June 30, 2000, which included a six-month automatic
extension through December 31, 2000 .
At the time Contract #24-933-6 (as amended by Contract Amendment
Agreements #24-933-8 and #24-933-9) was negotiated, the payment
limit was based upon targeted levels of utilization. However, the
utilization of services has exceeded the Department ' s projections
and during the negotiations, the California Department of Mental
Health revised rates for Long Term Care providers effective August
1, 2000 .
Approval of Contract Amendment Agreement #24-933-10 will amend the
payment provisions, to increase the automatic extension payment
amount and allow Contractor to continue providing services through
December 31, 2000 .

RECOMMENDATIONS:
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-251-3 with
HWW Inc. (dba Ad Club Advertising Services) effective October 15,
2000, to amend Contract #23-251 (as amended by Contract Extension
Agreement #23-251-1 and Contract Amendment Agreement #23-251-2) to
increase the Contract Payment Limit by $50, 000, from $335, 000 to a
new Contract Payment Limit of $385, 000 .
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Enterprise
I budget .
BACKGROUND/REASON(S) FOR RECOMMENDATION(5) :
On January 19, 1999, the Board of Supervisors approved Contract
#23-251 (as amended by Contract Extension Agreement #23-251-1 and
Contract Amendment Agreement #23-251-2) with Ad Club Advertising
Services, for the period from December 31, 1999 through December 31,
2000, to provide advertising services for the Department ' s Personnel
Office. On July 1, 2000, Ad Club Advertising Services incorporated,
using the new name of HWW Inc. (dba Ad Club Advertising Services) .
Approval of Contract Amendment Agreement #23-251-3 will allow the
Contractor to provide additional hours of advertising services
through December 31, 2000 .

Approve and authorize the Health Services Director or his designee
(Donna Wigand) to execute on behalf of the County, Novation Contract
#24-360-19 with Mental Health Consumer Concerns, Inc. , in an amount
not to exceed $134, 103 , to provide a CommunitySupport Worker
Project, a self-help program for "The Contra Costa Network of Mental
Health Clients" , and operation of community centers and a resource
library, for the period from July 1, 2000 through June 30, 2001 .
This Contract includes a six-month automatic extension through
December 31, 2001 in an amount not to exceed $67, 051 .
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment . This Contract
includes the Fiscal Year 2000-2001 Cost of Living Adjustment
authorized by the Board of Supervisors on June 27, 2000 .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S�:
This Contract meets the social needs of County' s population in that
it provides self-help programs for County' s mental health clients
and provides a mental health resource library/clearinghouse for
County residents.
On December 14, 1999, the Board of Supervisors approved Contract
#24-360-17 (as amended by Administrative Amendment Agreement
#24-360-18) with Mental Health Consumer Concerns for the period from
July 1, 1999 through June 30, 2000 (which included a six-month
automatic extension through December 31, 2000) to provide a self-
help program for the Contra Costa Network of Mental Health Clients,
operate the Contra Costa Community Center and continue the Consumer/
Community Support Worker Project .
Approval of Novation Contract #24-360-19 replaces the six-month
automatic extension under the prior Contract and allows the
Contractor to continue providing services through June 30, 2001 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Patrick Godley) to execute on behalf of the County, Contract
Amendment Agreement #23-267-2 with Epstein Becker & Green, effective
September 1, 2000, to amend Contract #23-267 (as amended by Contract
Amendment Agreement #23--267-1) , to amend the payment provisions and
extend the term of the Contract through November 30, 2001 .
FINANCIAL IMPACT:
Contractor shall be paid a fee rate of not less than $95 . 00 per hour
nor more than $325 . 00 per hour in accordance with the Contractor' s
usual and customary hourly billing rates for its services, as
approved by the Health Services Director or his designee. The
Contractor will also be reimbursed for travel on behalf of the
County, as approved in advance by the Health Services Director or
his designee, and for miscellaneous expenses actually incurred in
the provision of services under the Contract .
This Contract is funded by Enterprise I in the Health Services
Department ' s budget .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
On November 16, 1999, the Board of Supervisors approved Contract
#23-267 (as amended by Contract Amendment Agreement #23-267-1) with
Epstein Becker & Green, for the period from December 1, 1999 through
November 30, 2000, to provide the Department with temporary
specialized legal services regarding Managed Care issues, including
the Medi-Cal Local Initiative Health Plan contract.
Approval of Contract Amendment Agreement #23-267-2 will allow the
Contractor to provide services for ongoing litigation concerning
Health Plan rate issues, through November 30, 2001.
