FISCAL IMPACT: Approval of this Agreement will result in a total payment to Contra Costa County of $109, 500 . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On October 26, 1999, the Board of Supervisors approved Contract #29- 506-1 with County of San Mateo, for the period from July 1, 1999 through June 30, 2000 , for the provision of mental health treatment services for adolescents at the Fred Finch Youth Center. Standard Contract #24-920 with Fred Finch Youth Center provides intensive day treatment program and medication support services for seriously emotionally disturbed children (dually diagnosed) who are enrolled in the Fred Finch Youth Center Residential/Day Treatment Programs . Contra Costa County is acting as the host County for the Fred Finch Program, therefore, counties needing services will need to contract with Contra Costa for those services . Approval of Contract #29-506-2 will allow San Mateo County to continue paying Contra Costa County $100 per day for bed usage at the Fred French Youth Center through June 30, 2001 . FISCAL IMPACT: Approval of this Amendment will result in additional $25, 000 in Federal funding through the State for the WIC Program. No County funds are required. BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) : For over eighteen years the County has participated in the WIC Program with the State. This is a mandated program under the Community Health Services Division of the State Department of Health Services . WIC is a nutrition education, counseling and food supplement program for low-income, pregnant, postpartum and breast- feeding women, infants and children at nutritional risk. Approxi- mately 16, 550 clients are served by this program. Approval of Standard Agreement (Amendment) #29-203-77 will provide additional funding for the WIC Program, through September 30, 2002 . Four certified/sealed copies of this Board Order should be returned to the Contracts and Grants Unit . Department of Agriculture SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D. ) , to execute, on behalf of the County, Cooperative Agreement Amendment #28-562-6 (Federal Grant #58-3198-08- 031, Modification #2) with the U.S . Department of Agriculture (USDA) , Food and Consumer Service (formerly: Food and Nutrition Service) for the period from September 28, 2000 through September 27, 2001 . This is the third year funding of a four year Cooperative Nutrition Education/Food Wise Project . FINANCIAL IMPACT: Approval of this amendment will result in $89, 114 of USDA' s funding for the third year of a four year project for the Cooperative Nutrition Education/Food Wise Project for a total project amount of $354 , 665 . No County match is required. REASONS FOR RECOMMENDATIONS/BACKGROUND The goal of the Cooperative Nutrition Education/Food Wise Project is to improve nutritional well-being and reduce risk for chronic disease among food stamp household members, school meal participants, and children participating in the Child Care Council ' s Child Care Food Program, through the establishment and maintenance of a cooperative nutrition education service . On November 3 , 1998, the Board of Supervisors approved Grant/Cooperative Agreement #28-562-4 (as amended by Amendment Agreement #28-562-5) with the U.S. Department of Agriculture, for the period from September 28, 1958 through September 27, 2002 , for the Collaborative Food Stamp Nutrition Education Program "On the Move with Foodwise" Project . Approval of this Agreement (Amendment) #28-562-6 will allocate funding for Fiscal Year 2000-2001 in an amount not to exceed $89, 114 . Four signed copies of the amendment and three certified/sealed copies of the Board Order should be returned to the Contracts and Grants Unit . BACKGROM/REASON(S) FOR RECOMMENDATION(S) : On April 18, 2000, the Board of Supervisors approved Contract #28-528-19 with the County of Alameda, as the fiscal agent for Ryan White CARE Act, Title I funds, for coordination of services to Contra Costa residents with HIV disease and their families, for the period from March 1, 2000 through February 28, 2001. Approval of this Agreement (Amendment) #28-528-20 will provide additional funding for these services through February 28, 2001 . RECOMMENDATIONS: Adopt a resolution allowing the Health Services Director, or his designee, to apply for, and if approved receive EHAP Vill funding grant in an amount up to $50,000. (Resolution No. 2000/516) II. FISCAL IMPACT: The funds requested of the Emergency Housing Assistance Program (EHAP) Round Vill is essential to providing adequate emergency housing and supportive services to families year-round. No additional County funds are required. 111. BACKGROUND: According to the Continuum of Care Caps Analysis 2000, there are approximately 282 emergency shelter beds available for families in Contra Costa, 90 of which are only available during the cold weather season of November through March. The demand for shelter far exceeds the County's current capacity forcing homeless service providers to turn away an average of 40 families per night, many of which are from Central and East County where there are no year-round facilities. It is with this knowledge that the Contra Costa County Homeless Continuum of Care 5 yr. Plan (1996), approved by the County Board of Supervisors, identified the need for the expansion of year-round emergency shelter beds for families and ranked it a high priority. In August 2000, construction on a new transitional housing facility for homeless families began with the approval and support of the Contra Costa County Board of Supervisors and the Department of Housing and Urban Development. Aimed at creating additional supportive housing units in the County for parents and their children, the East County Transitional Family Center will offer families currently receiving services at the County's old transitional housing facility in Martinez, CA to move to the new state of the art facility in Antioch in April 2001. This expansion in transitional housing units creates a unique opportunity and creates a space for a year-round emergency shelter for 5-8 families at a time (24 beds) and fill a much need gap in emergency services. Anticipating a complete conversion to emergency services in June 2001, the Mt. View Emergency Shelter for Families located in Martinez, CA will provide shelter and supportive services to approximately 96 family members annually. Designed to meet the immediate shelter needs of parents and their children, Mt. View will be a year- round, 24-hour facility aimed at stabilizing each family by offering a safe environment with critical support services over a 90-day period. Every family will meet with a case manager to identify the short-term need's of each family member that will enable the family to move to a more stable living situation. These supportive services are the building blocks necessary to assist families in making the transition off the streets and into more stable housing. If the grant application authorized by this Resolution is approved, the Contra Costa Health Services hereby agrees to use the EHAP funds for eligible activities in the manner presented in the application as approved by the Department and in accordance with the program statute (Health and Safety Code Section 50800--50806.5) and Regulations (Title 25,Division 1, Chapter 7, Subchapter 12, Sections 7950 through 7976 of the California Code of Regulations), and Chapter 52, Statutes of 2000, and the Standard Agreement.
