S. Department of Veterans Affairs SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee (Frank Puglisi, Jr. ) , to execute on behalf of the County, Amendment #26-345-5 (Modification #1/#V261S-0290) with the U.S. Department of Veterans Affairs Northern California Health Care System (VANCHCS) to extend the term of the agreement from October 1, 2000 through September 30, 2001. FINANCIAL IMPACT: This Contract is funded in the Health Services Department ' s Enterprise I Budget . Under the terms of the agreement VANCHCS will be paid a fee for service in accordance with the attached fee schedule which is incorporated in the Contract . The services provided for the County' s patients under this Contract are billable to patients and third party payors. REASONS FOR RECOMMENDATIONS/BACRCROUND: On March 7, 2000, the Board of Supervisors approved Contract #26--345-4 with the VANCHCS, for the period from January 1, 2000 through September 30, 2001, to provide neurology services to County' s patients referred for treatment by Contra Costa Regional Medical Center (CCRMC) . Approval of this Amendment #26-345-5 (Modification #1/#V261S-0290) , will allow Contractor to continue providing services at the rates specified in the attached fee schedule, through September 30, 2001 . The Contract documents have always been prepared by the Veterans Administration. This contract includes a six-month automatic extension through December 31, 2001, in an amount not to exceed $109,146. FISCAL IMPACT: Funding is provided through an allocation from the Federal Perinatal Pilot Project. No County match is required. CHILDREN'S IMPACT STATEMR=: This Alcohol and Drug Abuse program supports the Board of Supervisors' "Families that are Safe, Stable, and Nurturing" community outcome by providing parenting education, stability, and safety for mothers (and their children) and pregnant women who are alcohol and drug dependent, while they are in perinatal Case Management program. Expected outcomes include delivery of drug-free babies, decreased use of alcohol, tobacco and other drugs, reduction in the number of relapses, and creation of a sober social network. BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : This Contract meets the social needs of County's population in that it provides case management services to pregnant and parenting women, in order to prevent perinatal substance abuse and improve birth outcomes. On January 18, 2000, the Board of Supervisors approved Novation Contract #24-618-10 (as amended by Administrative Amendment Agreement #24-618-11) with East Bay Perinatal Council for the period from July 1, 1999 through June 30, 2000 (which included a six-month automatic extension through December 31, 2000) for the provision of perinatal case management services for pregnant and postpartum substance abusing women and their children. Dr. Fouts practice has merged into a new provider group and in accordance with Paragraph 4 . (Termination) of the Contract, the Department and the Contractor have agreed to a mutual cancellation of this Contract. Approval of Contract Cancellation Agreement #26858-18 will accomplish this termination. Effective November 1, 1999, the Urology services provided by Dr. Fouts will be continued under Contract #26-977 with Pacific Urology.
