SUBJECT: APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to
AMEND contract (#21-304-I1) with Center for Human Development for Family Preservation
and Support (FPSP) services, to increase the contract amount by $9,200 from $195,252 to
$204,452 with no change in contract term.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to AMEND
contract (#21-304.11) with Center for Human Development for Family Preservation and Support (FPSP)
services, to increase the contract amount by $9,200 from $195,252 to $204,452 with no change in contract
term.
FINANCIAL IMPACT:
There is no County cost. Additional program funding is from a reallocation within the original Federal
Family Preservation and Family Support Act funding for fiscal year 1999/2000.
CHILDREN'S IMPACT STATEMENT:
This contract supports all five community outcomes: 1) "Children Ready for and Succeeding in School";
2)"Children and Youth Healthy and Preparing for Productive Adulthood"; 3)"Families that are Economically
Self-Sufficient"; 4)"Families that are Safe, Stable and Nurturing"; and 5) "Communities that are Safe and
Provide a High Quality of Life for Children and Families." Quality childcare services support all of the
outcomes established in the Children's Report Card. Children directly benefit from the care they receive each
day in a nurturing environment, while their parents can pursue employment, education, and other
opportunities.
BACKGROUND:
The Board approved the three-year Family Preservation and Support Program (FPSP) plan on June 15, 1999,
as approved by the Executive Committee of the policy forum.
The Employment and Human Services Department (EHSD) will continue the FPSP program through its
collaboration of community-based providers. Each collaboration focuses on developing and strengthening
community resources. The amended contract provides for additional strengthening family ties and conflict
resolution workshops for West County residents, with emphasis on serving North Richmond residents.

Center for Applied Local Research
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Donna
M. Wigand) to execute on behalf of the County, Contract #24-8982 with
Center for Applied Local Research, for the period from October 1, 2000
through June 30, 2001, in an amount not to exceed $35, 000, for the provision
of evaluation services for the Dual Diagnosis Demonstration Project. This
Contract includes an automatic three-month extension through September 30,
2001, in an amount not to exceed $15, 000 .
FISCAL IMPACT:
This Contract is 100% funded by a Federal Dual Diagnosis Grant {Substance
Abuse and Mental Health Services Administration (SAMHSA) .
No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATIONS)
On May 18, 1999, the Board of Supervisors approved Contract #24-898-1 with
Center for Applied Local Research, for the period from April 1, 1999 through
September 30, 2000 for the provision of evaluation services for the Dual
Diagnosis Demonstration Project .
Approval of Contract #24-898-2 will allow the Contractor to continue
providing services through June 30, 2001 .

regard to administrative analysis and program monitoring for the
Children' s Mental Health System of Care.

This Contract is 100% funded by Mental Health Realignment .
BACKGROUND}REASON(S) FOR RLCOMMENDATIONjS) :
Under Contract #74-080, Contractor will provide consultation, and
technical assistance to the Department with regard to the Children' s
Mental Health System of Care as the Program Monitor through August
31, 2001. Contractor' s activities shall include, but are not
limited to, providing administrative services, fiscal and program
contract monitoring, and acting as liaison between Contra Costa
Mental Health and Children' s Mental Health Community based
Contractors.

termination of Medical Specialist Contract #26-910-10 as amended by

2000 .

On September 21, 1999, the Board of Supervisors approved Contract #26-
910-10 (as amended by Administrative Amendment Agreement #26-510-11)
with Violet Smallhorne, M.D. , for the provision of professional
services in Contractor' s medical specialty, Plastic Surgery services,
for the period from August 1, 1995 through July 31, 2002 .
The purpose of this Board Order is to advise the Board that the
Department and the Contractor, have mutually agreed to terminate
Medical Specialist Contract #26-910-10 (as amended by Administrative
Amendment Agreement #26-910-11) , effective September 30, 2000 .

#25-013-3 with James Acuna, in an amount not to exceed $41, 876, for the
period from November 1, 2000 through October 31, 2001, to provide support
services for the Health Care to the Homeless Mobile Medical Team.
FISCAL IMPACT:
This Contract is 100% funded by the 330 (h) Homeless Grant .
BACKGROUND/REASON(S) FOR RECOMMENDATIONtS) :
On October 26, 1999, the Board of Supervisors approved Contract #25-013-2
with James Acuna, for the period from November 1, 1999 through October
31, 2000, for the provision of support services for the Health Care to
the Homeless Mobile Medical Team.
Approval of Contract #25-013-3 will allow the Contractor to continue
providing services through October 31, 2001 .
