SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION
Approve and authorize the Health Services Director or his Designee
(Wendel Brunner) , to execute on behalf of the County, Interagency
Agreement #28-641-1 with the Contra Costa County Office of Education
(Superintendent of Schools) , in the amount of $14, 000, for the period
from September 1., 2000 through June 30, 2001, for the Junior Health
Facilitator Project in West County.
FINANCIAL IMPACT :
Approval of this Interagency Agreement will result in $14, 000 to
support the Junior Health Facilitator Project through June 30, 2001.
No County funds are required.
REASONS FOR- RECO MMENDATIONSIBACI t Rfl
On January 18, 2000 the Board of Supervisors approved Interagency
Agreement #28-641 with the Contra Costa County Office of Education to
implement the Junior Health Facilitator Project in West County for the
period from October 1, 1999 through June 30, 2000 .
This project joins Contra Costa Health Services, the Contra Costa
County Office of Education and the Neighborhood Youth Corps in a
cooperative effort to reduce the number of children who grow up in
fatherless homes as the result of teen pregnancies. Under the Junior
Health Facilitator Project, junior health facilitators and Teenage
Program (TAP) health facilitators conduct class presentations,
organize health-related projects and create teen support groups in
West Contra Costa middle and high schools .
Approval of Interagency Agreement #28-641.-1 will continue funding for
this project through June 30, 2001 .

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND.JUSTIFICATION
RECOXKENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #29-773-2 with County
of Sonoma, for the period from September 1, 2000 through August 31, 2001,
for Contra Costa Health Plan to provide Advice Nurse Services to County of
Sonoma Health Plan members . Contra Costa County will be paid a minimum
monthly payment of $1, 800, in accordance with the following fee schedule :
1 to 50 calls 10� per member per month (pmpm)
51 to 125 calls - 15� pmpm
125 to 200 calls - 22G pmpm
201 to 329 calls - 29� pmpm
330 to 362 calls - 31� pmpm
363 to 395 calls - 33� pmpm
396 to 428 calls - 35� pmpm
429 or more calls - 37� pmpm
FISCAL IMPACT:
For a fee the Contra Costa Health Plan will provide Advice Nurse Services
to County of Sonoma. The revenue generated by this Contract will be used
to offset the cost of Contra Costa Health Plan (Health Plan) Advice Nurse
services .
BACKGROUND/REASON(S) FOR RECOMKENDA`3:ION(S) :
On October 19, 1999, the Board of Supervisors approved Contract #29-773-1,
for the period from September 5, 1999 through August 31, 2000, for
provision of Advice Nurse services.
Approval of Contract #29-773-2 will allow the Health Plan to continue to
provide County of Sonoma Health Plan members with telephone advice nurse
services including, but not limited to, provide information with regard to
accessing urgent care services and give clinical advice, through August 31,
2001 .

The Employment and Human Services Department has been contracting with the State Department of
Education (SDE) for over 25 years to provide childcare services under the State General Fund Child Care
and Development Program. These funds serve families who receive Child Protective services, teen parents,
children at risk of abuse and neglect, children with special needs and low income families.

SPECIFIC REQUESTS}OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Wendel
Brunner, M.D. ) to execute on behalf of the County, Contract #22-378-15 with
East Bay Perinatal Council, in the amount of $228, 653 , to provide case
management services for the Black Infant Health Project, for the period from
July 1, 2000 through June 30, 2001.
rISCAL IMPACT
This Contract is funded 100% by the State Department of Health Services,
Maternal and Child Health Branch, for the Black Infant Health Project. No
County funds are required.
CHILDRENIS IMPACT STATEMENT:
This Maternal and Child Health Program supports the County's "Children and
Youth Healthy and Preparing for Productive Adulthood" community outcome by
providing outreach to encourage pregnant women to receive prenatal medical
care and avoid behaviors that would increase the risk to their unborn
children. Expected programs outcomes include a decrease in the number of
babies being born with health problems requiring medical care.
BACRGROUND/REASON(S) FOR---.RECOMMENDATION(S) :
The Black Infant Health Project was created to provide services mandated by
the State Department of Health Services to develop a partnership for
reducing the African-American infant morbidity and mortality rates. This
Project meets the social needs of County's population by providing case
management services to African-American pregnant and/or parenting women and
their infants in West County.
On July 27, 1999, the Board of Supervisors approved Contract #22-378-14 with
the East Bay Perinatal Council, for the period from July 1, 1999 through
June 30, 2000. Approval of Contract #22-378-15 will allow the Contractor
to continue providing services through June 30, 2001.
