RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
#22-710-1 with Jena Fiel, in the amount of $41, 950, for the period
from February 1, 2000 through January 31, 2001, to provide
consultation and technical assistance to the Department with regard
to the Chronic Disease Prevention Organizing Project .
FISCAL, IMPACT
This Contract is funded by State Department of Health Services and
Private funding through a Grant from the Asian Pacific Health
Network (APHN) .
BACKOROIRM11REASON(S) FOR RECOMMENDATIONS)
The goal of the Chronic Disease Prevention Organizing Project is to
reduce the impact of chronic disease risk factors particularly in
ethnically diverse, low income communities, and to increase the
capacity of the local health department to use community organizing
methods, asset mapping, and other public health approaches to
address chronic disease.
1n September, 1999, the County Administrator approved and the
Purchasing Services Manager executed Contract #22-710 with Jena
Fiel, for the period from August 1, 1999 through January 31, 2000,
for the provision of consultation and technical assistance to the
Department with regard to the Chronic Disease Prevention Organizing
Project .
Approval of Contract #22-710-1 will allow Contractor to continue
providing services through January 31, 2001 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee (Frank
Puglisi, Jr) to execute on behalf of the County, Affiliation Amendment
Agreement No. 95-07829-A3 (County #29-607-6) with the Regents of the
University of California, to extend the term of the Agreement through March
31, 2000, for the provision of clinical experience at the County' s Health
Services facilities for students, residents and fellows of U.C. Davis
School of Medicine .
FINANCIAL IMPACT:
This is a non-financial agreement .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
On February 13 , 1996, the Board of Supervisors approved Affiliation
Agreement Contract #29-607-3 with the Regents of the University of
California (U.C. Davis School of Medicine (as amended by Contract Amendment
Agreements #29-607-4 and #29-607-5) , for the period from January 1, 1996
through December 31, 1999, to provide qualified training and clinical
experience at County Health Services facilities for medical students,
residents, family nurse practitioners, and other such students enrolled at
the University.
Approval of Affiliation Amendment Agreement #29-607-6 provides continuation
of training and clinical experiences at the County' s Health Services
Department facilities for students from U. C. Davis ' School of Medicine
through March 31, 2000 .
These documents have always been prepared by' the Regents of the University
of California (U.C. Davis) .

Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Unpaid
Student Training Agreement #22--610-1 with the Trustees of the
California State University on behalf of Sonoma State University,
for the period from February 1, 2000 through January 31, 2003, for
the provision of field instruction for the University's nursing
students.
FISCAL IMPACT:
None.
BACXGROUNDIREASON(S) FOR RECOMMENDATION(S) :
The purpose of this agreement is to provide Contractor's nursing
students with the opportunity to integrate academic knowledge with
application skills and attitudes at progressively higher levels of
performance requirements and responsibility. Supervised field work
experience for students is considered to be an integral part of both
the educational and professional preparation. The Department can
provide the requisite field education, while at the same time,
taking advantage of the students' services to patients.
On February 25, 1997, the Board of Supervisors approved Unpaid
Student Training Agreement #22-610 with the Trustees of the
California State University on behalf of Sonoma State University for
the period from February 1, 1997 through January 31, 2000.
Approval of Unpaid Student Training Agreement #22-610-1 will allow
students enrolled at Sonoma State University to continue to receive
supervised clinical experience through January 31, 2003 .

REASONS FOR RECOMMENDATIONSIBACKGROUNI):
On December 12 , 1995, the Board of Supervisors approved the County' s
Emergency Medical Services (EMS) Plan to provide a comprehensive
system within Contra Costa County to assure the best possible care
for injured or ill children.
Under Contract #23-273, Lilah Param, R.N. , will provide consultation
and technical assistance to the Department with regard to
administration, planning, developing, and implementation of the
Emergency Medical Services for Children (EMSC) Program through
September 30, 2000 .

RECOMMENDATION(8) :
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Contract
024-996-1 with Learning Services Corporation, for the period from
January 1, 2000 through June 30, 2000, in the amount of $100, 100,
for operation of a neurorehabilitation residential treatment program
for adults with Acquired Brain Injury Syndrome.
FISCAL IMPACT:
This Contract is 100% funded by Mental Health Realignment.
BACKGROUND TREASON(S) FOR RECOMMENDATION(S 1 :
Learning Services Corporation provides Supported Living Programs
(SLP) for catastrophically injured individuals who require 24-hour
care and supervision following brain injury. This Contract meets
the social needs of County's population by providing treatment to
mentally-impaired individuals that pose danger to themselves or
others, but who are not mentally ill.
On July 27, 1999, the Board of Supervisors approved Contract #24-996
with Learning Services Corporation for the period from April 1, 1999
through December 31, 1999 for the provision of a neurorehabilitation
residential treatment program for adults with Acquired Brain Injury
Syndrome.
Approval of Contract #24-996-1 will allow the Contractor to continue
providing services through June 30, 2000.

CHILDREN'S IMPACT STATEMENT:
This Mental Health program supports the following Board of Supervisors'
community outcomes : "Children Ready For and Succeeding in School" and
"Families that are Safe, Stable, and Nurturing" . Expected program
outcomes include an increase in positive social and emotional
development, as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS) and a decreased use of acute mental health care .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
In August, 1997, the State of California Legislature passed AB 1542 which
brought major changes to the welfare programs previously operated by the
State . Among the changes was a provision which required treatment of
mental illnesses of Welfare-to-Work participants when the condition
interferes with participation in Welfare-to-Work activities .
Subsequently, the County' s Employment and Human Services Department and
Health Services Department signed an Interdepartmental Services Agreement
(#21-427) which allowed the Health Services Department to provide mental
health services to referred Welfare-to-Work participants . Responsibility
for these mental health services involves contracts with organizational
providers to delivers these services .
Under Contract #74-035, Early Childhood Mental Health will provide mental
health services to recipients of the Cal-WORKS Program and their
children, through June 30, 2000 .

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Department Director, or his designee, to

FINANCIAL, IMPACT:
No County Costs. One hundred percent Federal Job Training Partnership Act funds.
CHILDREN'S IMPACT STATEMENT:
This action will directly support the third outcome, "Families that are Economically Self-Sufficient".
Activities under this contract provide employment opportunities to Job Training Partnership Act participants.
BACKGROUND:
On July 27, 1999 the Board authorized execution of a contracts (Item C.92) with various service providers for
Job Training Partnership Act clients in East County through the One-Stop Career Centers.
Liberty Union School District is one of the service providers identified in this action. The funds used to amend
the contract with Liberty Union School District come from the unspent funds from laskaogram year.

SUBJECT: APPROVE and AUTHORIZE the Employment & Human Services Department Director, or his
designee, to AMEND contract (#19-1268-0) with Antioch Unified School District raising the
payment limit by $6,817 to a new payment limit of$34,618 with no change in contract term.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Department Director, or his designee, to
AMEND contract (#19-1268-0) with Antioch Unified School District raising the payment limit by $6,817 to a
new payment limit of$34,618 with no change in contract term.
FINANCIAL IMPACT:
No County Costs. One hundred percent Federal Job Training Partnership Act funds.
CHILDREN'S IMPACT STATEMENT:
This action will directly support the third outcome, "Families that are Economically Self-Sufficient".
Activities under this contract provide employment opportunities to Job Training Partnership Act participants.
BACKGROUND:
On July 27, 1999 the Board authorized execution of a contracts (Item C.92) with various service for Job
Training Partnership Act clients in East County through the One-Stop Career Centers.
Antioch Unified School District is one of the service providers identified in this action. The funds used to
amend the contract with Antioch USD come from the unspent funds from last,progr ear.
