FISCAL IMPACT: This Contract is funded by Contra Costa Health Plan member premiums . Costs depend upon utilization. As appropriate, patients and/or third party payors will be billed for services . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : The Contra Costa Health Plan (Health Plan) has been obligated to provide professional optician services, including eyewear, for Health Plan patients with optician services as a covered benefit . On March-23 , 1999, the Board of Supervisors approved Contract #27-056-8 with Robert Turcios, O.D. , for the period from January 1, 1999 through December 31, 1999, for provision of optician services for Health Plan members . Approval of Contract #27-056-9 will allow the Contractor to continue to provide services, through December 31, 2000 . The maximum payment for combined lenses and frames OR contacts lenses is $65 . 00 per year, per commercial Health Plan member. C . Vision Exams are limited to one per year, per member. d. Contractor may charge the commercial Health Plan member the difference between the retail price of the vision care product and the maximum payment set forth above . Before determining the member' s share of cost, Contractor shall discount the regular retail price of the vision care product by at least 10 percent . Contractor shall not preclude Health Plan members from buying products advertised by Contractor at a discount or sales price which is greater than 10 percent . 2 . For Medi-Cal Members, County shall pay Contractor those rates set for in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 for like services and products*, once every two (2) years from the last date of services . In the event rate increases are subsequently approved by the State of California and are included in the County' s Health Plan capitated payment, County will thereafter increase the rates County pays to Contractor accordingly. *Contact lenses are covered if medically necessary and authorized by the Contra Costa Health Plan. 3 . For all other members of the Contra Costa Health Plan, County shall pay Contractor those rates set for in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 for like services and products . TO: BOARD OF SUPER= ORS William Walker, M.D. , Health Services Director FROM: By: Ginger Marieiro, Contracts Administratorowi Contra Costa DATE: January 26, 2000 County SUBJECT: Approval of Cancellation Agreement #24-086-113 (4) Winnie Harrison (dba Harrimon House) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)R BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee (Donna Wigand) , to execute on behalf of the County, Cancellation Agreement #24-086- 113 (4) with Winnie Harrison (dba Harrimon House) , effective December 31, 1999 . FINANCIAL IMPACT: Under this program, the County pays the State--established rates for the residential care of mental health clients who are not vet receiving SSI/SSP (but who are potentially eligible and have made formal application to the Social Security Administration for SSI/SSP) , contingent upon the client ' s signing an agreement with the County to reimburse the County from the SSI/SSP money, once it is finally granted by the federal government (usually 3 to 6 months retroactively back to the month of application) . For those mental health clients who are not granted SSIISSP (i .e. , are found ineligible) , the County covers the residential care payments to the facility operators through the County' s established Mental Health. Program Funding. Initially, ten to twenty percent of the clients are found ineligible by the Federal Social Security Administration, depending on changes in Federal guidelines, and then some of these are ultimately found eligible through appeal processes. REASONS FOR RECOMMENDATIONS(BACKGROUND: On April 20, 1999, the Board of Supervisors approved Residential Placement Agreement #24-086-113 (3) with Winnie Harrison (dba Harrimon House) for the period from March 1, 1999 through February 29, 2000, to provide emergency residential care to mentally disturbed adults who are potentially eligible for SSIISSP. Contractor has informed the Department that due to closure of home, she will no longer provide the services or activities at the Harrimon House under the Residential Placement Agreement #24--086-113 (3) . As of December 31, 1999, County-referred clients of Harrimon House have been successfully placed in other residential care homes. Therefore, in accordance with Paragraph 4 . (Termination) of the Contract, the Department and Contractor have agreed to a mutual cancellation of this Contract . Approval of Cancellation Agreement #24-086-113 (4) will accomplish this termination. This document includes a six-month automatic extension through December 31, 2001, in the amount of $54, 000 . FINANCIAL IMPACT: This Contract is included in the Health Services Department budget and is funded by AB 2185 fees the County charges to businesses for the handling of hazardous materials. REASONS FOR RECOMMENDATIONS/BACKGROUND: Pursuant to the Hazardous Materials Release Response Plan and Inventory Program (AB 2185) , approved by the Board of Supervisors on April 28, 1987, the County Health Services Department ' s Environmental Health Division is responsible to ensure that each hazardous materials handler in Contra Costa County is inspected annually. The County has contracted with the City of Richmond Fire Department since January 1988 to provide certain AB 2185 inspections at County' s request . On March 17, 1998, the Board of Supervisors approved Contract #22-276- 11 (as amended by Contract Amendment Agreement #22--276-12) to provide AB 2185 inspections for the period July 1, 1997 through June 30, 1999 (which included a six-month automatic extension through December 31, 1999) . Novation Contract #22-276-13 replaces the six-month automatic extension under the prior Contract and will allow the City of Richmond to continue providing these services through June 30, 2001 . The training includes special Hazardous Materials training classes through the University of California Extension, the California Fire Academy, and the California Specialized Training Institute; Hazardous Materials First Responder classes through the community college; and in-house training by individual experts . On March 17, 1998, the Board of Supervisors approved Contract #22- 351-7 (as amended by Contract Amendment Agreement #22--351-8) for provision of hazardous materials training, for the period from July 1, 1997 through June 30, 1999 (which included a six-month automatic extension through December 31, 1999) . Novation Contract #22-351-9 replaces the six-month automatic extension under the prior Contract . REASONS FOR RECODUMNDATIONS/BACKGROUND: On April 28, 1987, the Board of Supervisors approved the Hazardous Materials Release Response Plans and Inventory Program, County ordinance and Fee Resolution, a program which is mandated by California Health and Safety Code, Division 20, Chapter 6 . 95, § 25500 (et sect. ) . A significant component of the program is the preparation of Hazardous Materials Emergency Response Programs. On March 17, 1998, the Board of Supervisors approved Contract #22- 402-5 (as amended by Contract Amendment Agreement #22-402-6) to maintain a Hazardous Materials Response Unit in West County, for the period from July 1, 1997 through June 30, 1999 (which included a six-month automatic extension through December 31, 1999) . Novation Contract #22-402-7 replaces the six-month automatic extension under the prior Contract and will allow the City of Richmond to continue providing these services through June 30, 2001 . Approval of Contract Amendment Agreement #24-995-1, will allow the Contractor to provide an additional level of psychiatric services to bilingual clients at the Pittsburg Adult Mental Health Clinic, through April 30, 2000 .
