SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Chuck Deutschman) to execute on behalf of the County, Contract Amendment
Agreement #24-872-8 , effective January 1, 2000, to amend Contract
#24-872-7 with Bi-Beit Corporation, to increase the Payment Limit by
$36, 887 from $216, 859 to a new total Payment Limit of $253 , 746 .
FISCAL IMPACT:
This Contract Amendment Agreement provides an additional $36, 887 of State
funding through the Employment and Human Services Department, for the
provision of additional services for CalWORKs mothers and their children.
CHILDREN'S IMPACT STATEMENT:
This Alcohol and Drug Abuse program supports the Board of Supervisors,
"Families that are Safe, Stable, and Nurturing" community outcome by
providing parenting education, stability, and safety for mothers (and
their children) and pregnant women who are alcohol and drug dependent .
Expected outcomes include delivery of drug-free babies, decreased use of
alcohol, tobacco and other drugs, reduction in the number of relapses,
and creation of a sober social network.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On January 25, 2000, the Board of Supervisors approved Contract #24-872-7
with Bi-Sett Corporation, for the period from July 1, 1999 through June
30, 2000 (which included a six-month automatic extension through December
31, 2000) , for the provision of a Perinatal Residential Treatment
Facility for women and their children and Residential Substance Abuse
Treatment Center for women.
Approval of Contract Amendment Agreement #24-872-8 will allow the
Contractor to provide additional parenting services for CalWORKs mothers
and their children, through June 30, 2000 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #22-353-19, effective February 1, 2000 to amend
Contract #22-353-17 (as amended by Contract Amendment Agreement #22-
353-18) with Desarrollo Familiar, to increase the payment limit by
$20, 000 from $69, 000 to a new Contract payment limit of $89, 000 .
FISCAL IMPACT:
This Contract is funded 100% by the State Department of Health
Services, Office of AIDS . No County match is required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(8) :
On June 15, 1999, the Board of Supervisors approved Contract
#22-353-17 (as amended by Contract Amendment Agreement #22-353-18)
with Desarrollo Familiar for the period from July 1, 1999 through
June 30, 2000, to provide HIV/AIDS education, prevention and testing
outreach services in West County.
Approval of Contract Amendment Agreement #22-353-19 will allow the
Contractor to provide additional services, including expanded
education and outreach services, through June 30, 2000 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee
(Donna Wigand, L.C.S.W. ) to execute on behalf of the County,
Contract #74-039 with SERVE, Inc. , in the amount of $363 , 192, for
the period from January 1, 2000 through June 30, 2001, for provision
of vocational rehabilitation training services for mental health
clients .
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment .
BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County' s population in that
it provides transitional work services for mental health clients who
have either been unemployed for a period of time and are re-entering
the labor market or for those who are entering the work force for
the first time. Contractor' s services shall include, but are not
limited to, developing and refining client' s work habits, skills,
behaviors, and attitudes which will allow them to compete for and
obtain community employment .
Under Contract #74-039, SERVE, Inc . will provide vocational
rehabilitation training services for County' s mental health clients,
through June 30, 2001 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt Camhi) ,
to execute on behalf of the County, Contract #27-188-1 with Gretchen Graves, M.D. ,
for the period from January 1, 2000 through December 31, 2001, for provision of
primary care services for Contra Costa Health Plan members, to be paid as follows :
1. For Medi-Cal Beneficiaries:
a. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 plus
5%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan capitated payment,
County will thereafter increase the rates County pays to Contractor accordingly.
b. County will pay a quarterly case management fee,as follows:
Panel Size* Quarterly Fee
1 to 499 $3.00 per beneficiary per quarter
500 to 999 $3.25 per beneficiary per quarter
1000 or more $3.50 per beneficiary per quarter
*Panel Size is the number of Medi-Cal beneficiaries receiving treatment by Contractor during each quarter as specified in the "CCHP's Community
Provider Network Primary Care Providers Compensation Plan".
2. For Healthy Family PEWam Members. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances
in effect on August 1, 1998 plus 10%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.
3. For Plan B Commercial Members. County will pay Physicians for covered services, those rates set forth in the Medi-Cal Schedule of Maximum Allowances in
effect on August 1, 1998 plus 20%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums . Costs depend
upon utilization.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On February 1, 1997, the Local Initiative for Medi-Cal managed care in Contra
Costa County was implemented. Local Initiatives are required to include
traditional Medi-Cal providers from the community in their provider networks .
On April 1, 1997, the Board of Supervisors approved Contract #27-188 with Gretchen
Graves, M.D. , for the period from February 1, 1997 through December 31, 1999, for
provision of primary care services .
Approval of Contract #27-188-1 will allow the Contractor to provide services to
Health Plan members, through December 31, 2001 .
