SUBJECT: Approval of Contract #27--200-3 with John Rostowski, M.D.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-200-3 with John
Rostowski, M.D. , for the period from January 1, 2000 through December 31,
2000, for the provision of professional psychiatric services for the Contra
Costa Health Plan, to be paid as follows :
1. $100.00 per inital 45-50 minute therapy session, and
2. 70.00 for each additional 45-50 minute therapy session.
FISCAL, IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On January 19, 1999, the Board of Supervisors approved Contract #27-200-2
with John Rostowski, M.D. , for the period from January 1, 1999 through
December 31, 1999, to provide psychiatric services to Health Plan members.
Approval of Contract #27- 200-3 , will allow the Contractor to provide
professional psychiatric services to Contra Costa Health Plan members,
through December 31, 2000 .

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)S BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee (Milt
Camhi) to execute on behalf of the County, Contract #27-139-4 with Antioch
Counseling, for the period from January 1, 2000 through December 31, 2000,
for the provision of professional outpatient psychotherapy services, to be
paid as follows :
$50.00 per 50 minute individual therapy session; and
$30.00 per individual in a 50 minute group therapy session.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums . Costs
depend upon utilization. As appropriate, patients and/or third party
payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Contra Costa Health Plan (Health Plan) has an obligation to provide
professional outpatient psychotherapy services for Health Ulan members with
mental health therapy services as a covered benefit . This population
includes Medi-Cal, Medicare, and Commercial members enrolled in the Health
Plan.
On January 5, 1999, the Board of Supervisors approved Contract #27-139-3
with Antioch Counseling, for the period from January 1, 1999 through
` December 31, 1999 .
Approval of Contract #27-139-4 will allow the Contractor to continue to
provide professional outpatient psychotherapy services, through December
31, 2000 .

SUBJECT: Approval of Contract #24-959-2 with Family Stress Center
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract
#24-959-2 with Family Stress Center, in the amount of $41, 309, for
the period from January 1, 2000 through June 30, 2000, for the
provision of mental health services, including individual, group,
and family collateral_ counseling, case management, and medication
management for CalWORKs participants .
FISCAL IMPACT:
This Contract is 100% funded by the State CalWORKs through the
Employment and Human Services Department.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
In August, 1997, the State of California Legislature passed Assembly
Bill 1542 which brought major changes to the welfare programs
previously operated by the State. Among the changes was a provision
which required treatment of substance abuse and mental illnesses of
Welfare-to-Work participants, when these conditions interfere with
participation in Welfare-to-Work activities. Subsequently, the
County' s Employment and Human Services Department and Health
Services Department signed an Interdepartmental Services Agreement
(#21-427) which allowed the Health Services Department to provide
substance abuse and mental health services to Welfare-to--Work
participants referred by the Employment and Human Services
Department .
Under Contract #24-959--2, Contractor will provide mental health
services, including individual, group and family collateral
counseling, case management, and medication management services for
CalWORKs participants to reduce barriers to employment through, June
30, 2000 .

SUBJECT: Approval of Contract Amendment Agreement #24-635 .12 with Family
Stress Center, Inc.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Chuck Deutschman) to execute on behalf of the County, Contract Amendment
Agreement #24-•635-12, effective January 1, 2000, to amend Contract
#24-635-11 with Family Stress Center, Inc. , to increase the Payment Limit
by $45, 203 from $41,707 to a new total Payment Limit of $86, 910 .
FISCAL IMPACT:
This Contract Amendment Agreement provides an additional $45, 203 of State
funding through the Employment and Human Services Department, for the
provision of additional services for CalWORKs mothers and their children.
CHILDREN'S IMPACT STATEMENT:
This Alcohol and Drug Abuse program supports the Board of Supervisors,
"Families that are Safe, Stable, and Nurturing" community outcome by
providing parenting education, stability, and safety for mothers (and
their children) and pregnant women who are alcohol and drug dependent .
Expected outcomes include delivery of drug-free babies, decreased use of
alcohol, tobacco and other drugs, reduction in the number of relapses,
and creation of a sober social network.
BACKGROUNDUIREASON(S) FOR RECOMMENDATION(S) :
On January 11, 2000, the Board of Supervisors approved Novation Contract
#24-635-11 with Family Stress Center, Inc . , for the period from July 1,
1999 through June 30 , 2000 (which included a six-month automatic
extension through December 31, 2000) , for the provision of parenting
consultant services for County-referred families participating in the
Born Free Project .
Approval of Contract Amendment Agreement #24-635 -12 will allow the
Contractor to provide additional services, including but not limited to,
the development and implementation of a parenting component for CalWORKs
mothers who have completed the Born Free Program, through June 30, 2000 .
