FINANCIAL IMPACT:
Per State Department of Education Agreement F3AP-9012 (Stage 3) County to receive funding in the amount of
$13,598.
BACKGROUND:
The Employment and Human Services Department has been contracting with the State Department of
Education (SDE) for over 25 years to provide child care services under the State General Fund Child Care
and Development Program. These funds serve families who receive Child Protective Services, teen parents,
children at risk of abuse and neglect, children with special needs and low income families.

California, Department of Alcohol and Drug Programs

RECONMNDECD ACTION:
Approve and authorize the Health Services Director or his designee (Chuck Deutschman) to execute,
on behalf of the County, Standard Agreement (Amendment) #29-502-3 (State #SCC07 (99/00) , Al) with
the State Department of Alcohol and Drug Programs, to amend the amount of funding for fiscal year
1999-2000 of this three year agreement effective July 1, 1998 through June 30, 2001, to receive
State General Fund, Federal Block Grant allocations and Drug/Medi-Cal monies to fund
approximately 72% of Community Substance Abuse Services.
FINANCIAL IMPACT:
Funding for this agreement is increased by $2,424,678 from State, Federal and Federal Drug/Medi-
Cal as indicated below:

The required County match for this Agreement is $110,514 and is included in the Departments
Fiscal Year 1999/2000 budget.
BACKGRO=/RRASON(S) FOR RRCObMNDATION(S) :
Standard Agreement (Amendment) #29-502-2, approved by the Board of Supervisors on August 17,
1999, was based on an initial allocation revision that is being subsequently revised in this
Standard Agreement (Amendment) #29-502-3, based on the current levels of funding. Like the
Standard Agreement, this amendment requires counties to provide Drug/Medi-Cal services up to
their full State General Fund allocation. If the required services exceed the allocation,
counties may access a Drug-Medi-Cal reserve set aside for this purpose. If the County declines
to provide Drug/Medi-Cal services the State will reduce the State General Fund allocation by up
to $2,146,786 and the County will lose the Federal Drug/Medi-Cal reimbursement of $1,410, 698,
for a total loss of up to $3,557,484, and will assign those funds to a contractor to provide the
services.
Approval of this Standard Agreement (Amendment) #29-502-3 will guarantee that the County will
continue to receive the full allocation amount included in the Agreement and allow the Department
to continue providing needed Substance Abuse Services.
Three sealed copies of this Board Order should be returned to the Contracts and Grants Unit for
submission to the State.

CORRECT Board Action of December 7, 1999 Item C.83. APPROVE and AUTHORIZE the
Employment and Human Services Director, or designee to enter into an MOU with the State
Health and Human Services Agency Data Center (#29-0358-2) in the CORRECTED amount of
$163,760.30 for the period from October 9, 1999 through October 8, 2001,
AND Item C. 75 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee to enter into an MOU with the State Health. and Human Services Agency Data Center
(#29-068-1) in the CORRECTED amount of $166,804.903 for the period from November 1 , 1999
through October 30, 2041 .
FINANCIAL IMPACT:
No County match is required. This agreement allows the services of Mr. Jack Rogers and the
services of Ms. Danielle Hunter to be fully reimbursed by the State Health and Human Services
Agency Data Center. 100% of salary and fringe benefits will be reimbursed by the State for
payment by the County to Mr. Rogers. The State will also directly reimburse Mr. Rogers and
Ms. Hunter for any travel and expense related to this project.
BACKGROUND:
The State has recruited staff from counties to develop and implement the statewide Child
Welfare System/Case Management System (CWS/CMS). The intent is to integrate county
experience into the CWS/CMS implementation, and to provide county staff with a system wide
perspective for implementation. This Board Order reflects the updated amounts involved in the
cost reimbursement agreement between Contra Costa County and the State Health and Human
Services Agency Data Center.

Approve and authorize the Health Services Director, or his designee, (Frank
Puglisi, Jr. ) to execute on behalf of the County, Contract Amendment Agreement
#26-963-1, effective November 15, 1999, to amend Contract #26-963 with Sharon
D. Washington, M.D. , (Specialty: Obstetric/Gynecology) , to modify the fee
schedule set forth in the Additional Provisions, to allow Contractor to be paid
for additional weekend and weeknight on-call coverage services.
FISCAL IMPACT:
This amendment will modify the fee schedule set forth in Contract ##26-963, Additional
Provisions, Paragraph 1. (Payment) , subparagraph a. , to read as follows:
°l.a. (1) $17,000 per month, for the provision of Obstetric and Gynecology services,
including consultation, clinical coverage, training, medical and/or surgical
procedures, and on-call coverage services defined as one (1) 14 hour weeknight
shift, per week and one (1) 24 hour weekend shift every 4th week) , NOT TO EXCEED
$204,000 annually; and
(2) 1n the event Contractor provides more than one (1) 14 hour weeknight shift, per
week and one (1) 24 hour weekend shift every 4th week of on-call coverage services,
at the request of County, County shall pay Contractor $61.90 per hour.
Cost to the County depends upon utilization. As appropriate, patients and/or third party payors
will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATIONS:
On September 21, 1999, the Board of Supervisors approved Contract #26-963 with
Sharon D. Washington, M.D. , for the period from September 15, 1999 through
September 30, 2002, for the provision of Obstetric/Gynecology services for
Contra Costa Regional Medical Center and Contra Costa Health Centers.
As the result of a extended medical leave of one full-time physician in the
Obstetric/Gynecologist Department at the CCRMC, the need for
Obstretic/Gynecology services and on-call coverage has increased. At the
request of the County, Dr. Washington has agreed to provide one additional
weekend and one additional weeknight of on-call coverage, in addition to her
regularly scheduled on-call coverage services for Contra Costa Regional Medical
Center and Contra Costa Health Centers .
Approval of Contract Amendment Agreement #26-963-1 will allow Dr. Washington to
be provide additional on-call services at County' s request .

Approve and authorize the Health Services Director or his designee (Frank

#26-959-1, to amend Medical Specialist Contract #26-959 with Joseph Macedo,
Jr. , M.D. (Specialty: Obstetrics/Gynecology) , effective November 15, 1999, to
modify the fee schedule set forth in the Additional Provisions, to allow
Contractor to be paid for additional weekend and weeknight on-call coverage
services .
FINANCIAL IMPACT:
This amendment will modify the fee schedule set forth in contract #26-959, Additional

"a. (1) $16,000 per month, for the prevision of Obstetric and Gynecology services,
including clinical coverage, medical and/car surgical procedures, consultation,
training, and on-call coverage services defined as (one (1) 14 hour weeknight
shift, per week and one (1) 24 hour weekend shift every 4th geek) , NOT TO EXCEED
$192,000 annually; and
(2) In the event Contractor provides more than one (1) 14 hour weeknight shift, per
week and one (1) 24 hour weekend shift every 4th week, of on-call coverage
services, at the request of County, County shall pay Contractor $61.90 per hour. "
Cost to the County depends upon utilization. As appropriate, patients and/or third party

REASONS FOR RECCIMWE=ATIQ} SIBACKQFtQM:
On April 6, 1999, the Board of Supervisors approved Medical Specialist Contract
#26--959 with Joseph Macedo, Jr. , M.D. , for the period from April 1, 1999
through March 31, 2002, for provision of Obstetric and Gynecology services for
Contra Costa Regional Medical Center and Contra Costa Health Center.
