Notice of Completion of Contract with and NOTICE OF COMPLETION
RJJ, Inc. (C.C. §3086, 3093} ZOOo/74
Budget Line Item.No. 4405-4264 RESOLUTION NO.
The Board of Supervisors RESOLVES THAT:
The County of Contra Costa on June 22, 1999, contracted with RJJ, Inc., for Reroofing at the
Court House, 725 Court Street, Martinez, Budget Line Item No. 4405-4264, Authorization
No. 0928-WH264F, with National American Insurance Company as surety, for work to be
performed on the grounds of the County; and
The Director of General Services reports that said work has been inspected and complies with
the approved plans and specifications, and recommends its acceptance as complete as of
February 15, 2000; and recommends the withholding of$7,500.00 pursuant to a letter dated
February 1, 2000;
Therefore, said work is accepted as recommended above, and the Clerk shall file with the
County Recorder a copy of this Resolution and Notice as a Notice of Completion for said
contract; and the Director of General Services is directed to withhold monies otherwise due
pursuant to the letter dated February 1, 2000.
Time extension to the date of acceptance is granted as the work was delayed due to
unforeseeable causes beyond the control and without the fault or negligence of the
Contractor.

The Board of Supervisors RESOL'V'ES THAT:
The County of Contra Costa on July 20, 1999, contracted with Blythe & Associates General
Contractor, for Restroom Accessibility Improvements at El Sobrante Library, 4191 Appian
W4 y, El Sobrante, Budget Line Item No. 4405-4391,Authorization No. 0928-WH391B, with
United Pacific Insurance Company as surety, for work to be performed on the grounds of the
County, and
The Director of General Services reports that said work has been inspected and com lies with
the approved plans and specifications, and recommends its acceptance as complete as of
February 15, 2000; and recommends the withholding of$2,410.00 pursuant to a letter dated
January 31, 2000;
Therefore, said work is accepted as recommended above, and the Clerk shall file with the
County Recorder a copy of this Resolution and Notice as a Notice of Completion for said
contract; and the Director of General Services is directed to withhold monies otherwise due
pursuant to the letter dated January 31, 2000.

SUBJECT: Approving Plans and Specifications for
Cast Iron Soil Pipe Replacement at Los Medanos Health Center
2311 Loveridge Road,Pittsburg

WHEREAS the Board on November 16, 1999 directed the County Administrator and the
Director of General Services to initiate the process to replace the entire existing above-ground
cast iron waste pipe and vent system at the Los Medanos Health Center; and
WHEREAS the Board on February 1, 2000 approved the ninth amendment to a consulting
services agreement with Fong & Chan Architects, a major portion of which was to provide
design, construction documents, and construction administration for the replacement of the
existing above-ground cast iron waste pipe and vent system as the Los Medanos Health Center;
and
WHEREAS plans and specifications for Cast Iron Soil Pipe Replacement at Los Medanos
Health Center, 2311 Loveridge Road, Pittsburg, have been filed with the Board this day by the
Director of General Services; and
WHEREAS plans and specifications were prepared by Fong & Chan Architects, and
WHEREAS the cost estimate for the initial construction contract is $850,000.00; and
WHEREAS the Board has obtained the general prevailing rates of wages, which shall be
the minimum rates paid on this project; and
WHEREAS the Board has previously found on February 18, 1998 that the project is a
California Environmental Quality Act(CEQA) Class 1 a Categorical Exemption, and the Director
of Community Development has filed a Notice of Determination with the County Clerk.
IT IS BY THE BOARD RESOLVED that said plans and specifications are hereby
APPROVED. Bids for this work will be received on March 16, 2000 at 2:00 p.m., and the Clerk
of this Board is directed to publish a Notice to Contractors in accordance with Section 22037 of
the Public Contract Code, inviting bids for said work, said Notice to be published in
Centra Cos to 'Rimes . The Director of General Services is
directed to mail notices to the construction trade journals specified in Section 22036 of the
Public Contract Code at least 30 days before the date of opening the bids.
IT IS FURTHER. RESOLVED that the Director of General Services, or his designee, is
AUTHORIZED to issue bid Addenda, as needed, for clarification of the contract bid
documents, provided the involved changes do not significantly increase the cost estimate for the
initial construction contract.

WHEREAS plans and specifications for Tenant Improvements at 2366-A Stanwell Circle,
Concord for Department of Agriculture, have been filed with the Board this day by the Director
of General Services; and
WHEREAS plans and specifications were prepared by Daniel Rainey,Architect; and
WHEREAS the cost estimate for the initial construction contract is $290,000.00; and
WHEREAS the Board has obtained the general prevailing rates of wages, which shall be
the minimum rates paid on this project; and
WHEREAS the Board hereby DETERMINES that the project is a California
Environmental Quality Act (CEQA) Class la Categorical Exemption; DIRECTS the Director of
Community Development to file a Notice of Exemption with the County Clerk; and DIRECTS
the Director of General Services to arrange for payment of the $25.00 handling fee to the County
Clerk for filing the Notice of Exemption and a $25.00 handling fee to Community Development
for administration processing costs.
IT IS BY THE BOARD RESOLVED that said plans and specifications are hereby
APPROVED. Bids for this work will be received on March 23,2000 at 2:00 p.m., and the Clerk
of this Board is directed to publish a Notice to Contractors in accordance with Section 22037 of
the Public Contract Code, inviting bids for said work, said Notice to be published in
Contra Costa Times . The Director of General Services is
directed to mail notices to the construction trade journals specified in Section 22036 of the
Public Contract Code at least 30 days before the date of opening the bids.
IT IS FURTHER RESOLVED that the Director of General Services, or his designee, is
