SUBJECT: PILOT ON INCREASING PARTICIPATION IN HEALTH CARE AND PREVENTION BEHAVIORS.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
Recommendation:
1. RECOGNIZE that small incentives have been used successfully to encourage patients to take a
more active part in managing their health care.
2 RECOGNIZE that improved consumer participation has been shown to improve health outcomes.
3. AUTHORIZE the Health Services Director or his designee to conduct a twelve month pilot utilizing small
incentives to promote increased patient participation in management of chronic diseases and high risk
pregnancy.
4. APPROVE up to$20,000 in expenditures for the pilot on increasing participation in health care and
prevention behaviors.
5. REFER the pilot program to the Family and Human Services Committee for review of its cost-effectiveness.
Backwound:
CCHP is charged with improving members health status by providing access to primary care and preventive health
measures in order to avoid more costly and unnecessary emergency, specialty and/or tertiary care services. In order
to achieve the best outcomes, CCHP is requesting authority to use modest incentives as a way to increase
participation with ongoing and preventive care, such as immunizations, well child visits, prenatal care, and routine
screening tests. Additionally it is essential to address chronic health conditions in early stages while they are still
treatable and preventable. To maximize use of preventive health care services, the plan makes use of various
innovative strategies that go beyond typical curative care; health promotion activities, health education classes, case
management of high risk health conditions, and outreach activities that increase health awareness and promote
members' ability to take responsibility for their own health.

To further promote active and ongoing participation in these activities, it is customary and effective for health plans to
use incentives such as small rewards for completing certain educational requirements. For example, many health
plans offer an infant car seat as a reward for completing the full set of expected prenatal visits; others give small
baby items as rewards for attending educational classes. Chronic disease management programs that encourage
members to learn how to avoid emergency care by managing their own illness, such as, asthma or diabetes,
frequently use gift certificates in small denominations for use at a local grocery. Such programs have proven
successful in a variety of settings and are considered a sound investment because of the savings that result when
patients gain the knowledge and skills to prevent more serious complications by effectively managing their own care.
Solano county's Perinatal Case management program is very similar to the one CCHP is developing and includes the
use of incentives. They were able to decrease their rate of premature births by 60% (4.2% to 1.7%). This represented
a 92% savings on investment, primarily through decreased costs of Neonatal Intensive Care Unit days.
CCHP is seeking authority to pilot an incentive program. Incentives range from $10-$50 certificates for food or baby
supplies, with most certificates being for$25 or less. Each program offering incentives shall monitor the program
quarterly to assure that the money is being used as intended. No one individual shall receive duplicate incentives or
participate in more than 2 different incentive programs per year. The total cost of the program for a year will be less
than$20,000. This program shall be reviewed by the Chief Executive Officer CCHP,the Health Services Director, the
CCHP Joint Conference Committee and the Family and Human Services Committee.
