RECOMMENDATION(S) s
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract Amendment
Agreement #24-681-2 (10) with Susan Regal (dba God' s Grace) , to amend
Contract #24-681-2 (9) , effective September 15, 2000, to increase the
payment limit by $16, 730 from $72, 000 to a new total Contract Payment
Limit of $88,730 .
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Fiscal Year
2000-2001 Budget and is 100 funded by Mental Health Realignment
funding.
BACKGR LM/REASON(S) FOR RECOMMENDATION(S) :
On June 6, 2000, the Board of Supervisors approved Contract #24-681-
2 (9) with Susan Regal (dba God' s Grace) , for the period from July 1,
2000 through June 30, 2001, for the provision of augmented board and
care services for County-referred mentally disordered clients.
Approval of Contract Amendment Agreement #24681-2 (10) will allow the
Contractor to provide additional services to County-referred mentally
disordered clients through June 30, 2001 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) to execute on behalf of the County, Contract #27-350-3 with Julie O.
Fulford (dba Valley Audiology) , in an amount not to exceed $50, 000, for the
period from November 1, 2000 through October 31, 2001, for the provision of
professional audiological services for Contra Costa Health Plan members .
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan Member Premiums . As
appropriate, patients and/or third-party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On November 2 , 1999, the Board of Supervisors approved Contract #27-350-2
with Julie Fulford (dba Valley Audiology) for the period from November 1,
1999 through October 31, 2000, to provide audiological services to Contra
Costa Health Plan members who have been referred by County-designated
physicians .
Approval of Contract #27-350-3 will allow the Contractor to continue
providing services through October 31, 2001 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Milt Camhi) to execute on behalf of the County, Contract Amendment
Agreement #27-436-2 with Florence Chan, to amend Contract #27-436-1 ,
R.N. , effective November 1, 2000, to increase the Contract payment
limit by $7, 935, from $65, 480 to a new total of $73 , 415 .
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums .
Costs depend upon utilization. As appropriate, patients and/or third
party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S)_:
On May 16, 2000, the Board of Supervisors approved Contract #27-436-1
with Florence Chan, for the period from June 1, 2000 through May 31,
2001, to provide consultation and technical assistance to Contra Costa
Health Plan with regard to Authorizations and Utilization Management
Units .
Approval of Contract Amendment Agreement #27-436-2 will allow the
Contractor provide additional services including, but not limited to,
supervision and direction of the Health Plan' s Psychiatric Unit,
through May 31, 2001 .

RECOMMENDED ACTION:
Approve and ' authorize the Health Services Director, or his
designee (Donna Wigand) , to execute on behalf of the County,
Cancellation Agreement #74-008-2 with Sylvia E. Castillo, LCSW,
effective at the close of business on November 8, 2000 .
FIIANCIAL IMPACT
This Contract is 100% Federally funded.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
On September 26, 2000, the Board of Supervisors approved Contract
##74-008-1, with Sylvia E. Castillo, LCSW, for the period from
October 1, 2000 through September 30, 2001, to provide
Consultation and technical assistance to the Department with
regard to child and adolescent mental health programs .
The Contractor recently notified the Department that it wished to
terminate this contract . Therefore, in accordance with General
Conditions Paragraph 5. (Termination) of the Contract, the
Department and Contractor have agreed to a mutual cancellation of
this Contract . Approval of Cancellation Agreement #74-008-2 will
accomplish this termination.

Arnold Gilbert, M.D.

RECOMMMATI ON(S) :
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Mental Health
Specialist Contract #24-804-5 with Arnold Gilbert, M.D. , in an
amount not to exceed $126, 464, to provide professional psychiatric
services, for the period from October 1, 2000 through June 30, 2001 .
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment, and offset by
third party billing.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S)_:
For a number of years the County has contracted with Medical, Dental
and Mental Health Specialists to provide specialized professional
services which are not otherwise available in its hospital and
clinics .
On March 7, 2000 the Board of Supervisors approved Mental Health
Specialist Contract #24-804-4 with Arnold Gilbert, M.D. , for the
period from February 1, 2000 through January 31, 2001, for the
provision of psychiatric services to Central County Children and
East County Adults . County and Contractor have mutually agreed to
terminate the existing services under Contract #24-804•-4 and to
substitute Contract #24-804-5 in its place.
Approval of Contract #24-804-5 will reflect an increase in salary,
allowing Dr. Gilbert to continue providing services through June 30,
2001 .
