Backtrrourlld: In 1997 the Board of Supervisors established the Ombudsman Program in the Health Services Department to serve the members of CCHP and Evelyn Rinzler was appointed as the first ombudsperson. The ombudsman program last year produced a Contra Costa County Transportation Resources Directory of free and reduced fare transportation services in the county. The ombudsman program has just produced the Community Resource Directory describing health and human resources. The directory was intended originally for Contra Costa Health Plain(CCBP)members and is also helpful for other county residents and the agencies that work with them. Acknowledgements The Ombudsman Program extends special thanks to the many staff members of the Contra Costa Health Services Department and the Area Agency on Aging for their contributions of both ideas and research materials used in this book. Julie Kelley provided significant guidance during the planning stage, and Lee Ann Winton was the catalyst for the actual production of the directory. Above all deep gratitude is owed to Milt Camhi, without whose vision and support the book would never have come into being. The Ombudsman Program of Contra Costa Health Services is available to all members of Contra Costa Health Plan. The Ombudsman serves as a guide, helping patients navigate their way through the health care system. Direct correspondence to. Index Preface This directory was produced in reponse to a need in Contra Costa County for a useful book describing health and human resources that reflect a broad spectrum of peoples' lives. Intended originally for the members of the Contra Costa Health Plan, the directory should be helpful not only for county residents but also for agencies that work with people. In general the agencies listed herein are either non-profit, governmental, have no fees or a sliding scale,or accept Medi-Cal,Medicare and private insurance. The Ombudsman Program has made every effort to include information exactly as provided. Any mistakes on the part of the agencies, Contra Costa Health Services, or its printers, are inadvertent and sincerely regretted. Inclusion does not imply endorsement, nor does exclusion indicate anything other than the limitations of time and space. Due to the prevalence of changes in health law, such as Welfare, new agencies will arise and old ones disappear. This directory is current as of May, 1999,and will doubtless require updating in the future.
