SUBJECT: CDBG Public Service Proposal
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
RECOMMENDATIONLSI-
APPROVE and AUTHORIZE the County Administrator, or designee, to submit a proposal to the
Contra Costa County Community Development Department for $15,000 to fund Substance Abuse
Services as a component of the Service Integration Program.
BACKGROUND/REASON(S) FOR REQOMM.ENDATION(S):
The Service Integration Program provides integrated services of the County departments of Social
Service, Health Services and Probation at sites in Bay Point and North Richmond. Services provided
include: Income Maintenance, Child Welfare, Employment Services, Public Health, Mental Health,
Probation and linkage to public schools. This proposal would allow for the continued expansion of the
Service Integration collaborative to include Community Substance Abuse Services of the County
Health Services Department in partnership with New Connections, a substance abuse agency.
Substance abuse services are a critical component of the Integrated Services Team Program.Among
the 600 families served by SIT in Bay Point, and the 300 served in North Richmond and Old Town
San Pablo, an estimated 50 percent have problems of substance abuse., These problems spawn
other problems in the two communities, notably child abuse, unemployment and crime.
The CDBG proposal will provide second year funding for a part-time Substance Abuse Coordinator
to provide Bay Point, North Richmond and Old Town San Pablo SIT families with assessment and
short-term counseling, group counseling and substance abuse education. The Coordinator will also
provide consultation services to SIT staff, multiplying services to SIT families and the community at
a decreasing cost of services per person served.

SPECIRC REQUEST{S)OR RECOMMENDATIONS)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute, on behalf of the County,
Interagency .Agreement #29-658 with the Contra Costa County
Superintendent of Schools (Office of Education) , for the period from
July 1, 1998 through June 30, 1999, with a total payment limit
$9, 049, for the provision of professional medical consultation to
enhance the health of special needs students.
FINANCIAL IMPACT.-
Under
MPACT:Under this Agreement, the Department will invoice the CCC
Superintendent of Schools an amount not to exceed $9,900 .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Health Services Department and the County Office of Education
(Special Education Department) wish to join in a cooperative effort
to enhance the health for Special Needs Students of Contra Costa
County.
Under this Agreement, Contra Costa Regional Medical Center will
provide a qualified individual to serve as a ' member on the
individualized education planning team to assist in determining the
specialized health care needs of high-risk children, act as a
liaison between the school and the child' s private health care
provider and provide consultant services to school nurses concerning
children with exceptional health care needs and problems.
The Contra Costa County Superintendent of Schools (Office of
Education) will pay the Department a total of $9, 000 to provide
medical consultation services through June 30, 1999.

SUBJECT: California Healthcare Foundation Grant
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
Recommendation:
A. Accept the $75,000 California Healthcare Foundation grant for planning for the Medicare Managed
Care Partnership Program for the period October 6, 1998, through September 30, 1999; authorize the
Health Services Director and Social Service Director to collaborate in the planning efforts; and
authorize the Director, County Office on Aging, to execute the Grant Agreement and Conditions.
B. Authorize the Health Services Director or his designee and the Social Service Director or his designee
to submit by July 1, 1999, a full proposal to the Foundation for an Implementation grant under the
Foundation's Program for Elders in Managed Care.
Back ound:
Contra Costa Health Plan(CCHP),under a planning grant from the federal government(HCFA),has been
developing a Social HMO which combines acute and community based long-term care services in an
integrated managed care system. CCHP has submitted its operational protocols running;a S/HMO to HCFA
and has developed an extensive case management program which includes a state-of-the-art case
management computer program which is now being used for both elderly and high risk OB CCHP
members. Unfortunately, HCFA has delayed the implementation of the S/HMO: It has not released the
final S/HMO reimbursement methodology and probably will not do so until it solves its year 2000 computer
problems. HCFA is also delaying for at least a year its scheduled report to Congress regarding conversion
of S/HMO demonstration projects to permanent status.
In light of federal government's delaying of S/HMO implementation and because we remain committed
to serving the frail elderly,the Health Services Department is reviewing the current CCHP Medicare cost
contract to both increase its marketability and to add some community social support benefits. Recently
several Medicare risk plans, including Prudential, Lifeguard, and United are canceling their Medicare
contracts in Contra Costa County and several others have curtailed marketing,and we may be able to attract
some of these Medicare HMO members who may be looking for new HMO health coverage.
The Social Services Department is in the process of significantly expanding the availability of case
management services for both frail elders and younger disabled adults at risk of inappropriate or premature
institutionalization. As the result of recently adopted State legislation and State budget approval,the Social
Service Department',will be establishing both a Multipurpose Senior Services Program and a Linkages Case
Management Program in our county and broadly expanding Adult Protective Services. Collectively these
new program initiatives will greatly enhance the ability of the Social Service Department to access home-
and community-based services for frail elders and dependent younger adults.

The California Health Care Foundation grants offer another avenue for improving services to seniors with
special needs. The grant will assist the Health Services Department and the Social Services Department
to plan together to improve our systems of care for the elderly who have serious and persistent health care
needs and who are enrolled in managed care. If we later obtain an implementation grant offered by the
California Health Care Foundation, the two departments will then be able to meet the goals originally
established for the S/HMO program. We will be able to offer to frail and at-risk elderly an increased
emphasis on prevention and community-based care; a greater flexibility in services, and an enhanced
coordinated, integrated system of care.
Health Services and Social Service will continue to explore other collaborative strategies. Currently, staff
from both departments are evaluating the suitability of both departments working together as lead agencies
in the development of a comprehensive plan for long-term care in Contra Costa County under the California
Long-Term Care Pilot Program(AB 1040).
The HSD is also investigating the development of a risk-based long-term care pilot program. AB 2583,
Shelly,which was signed by the governor on September 13, 1998, allows for up to 10 such demonstration
projects in the state.
Fiscal Impact•
Immediate receipt of$75,000 planning grant and potential receipt of three-year implementation grant to
fund expansion of coordinating and delivering a full range of health and supportive services to sick and at-
risk seniors.

SUBJECT.
APPROVE and AUTHORIZE Director of Social Service, or designee, to apply to the Federal Transit
Administration for the purchase of shuttle vans to su ri trans station needs of CalWORKs artici ants.
SPECIFIC REOUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECON01ENDED ACTION:
APPROVE AND AUTHORIZE the Director of Social Service, or designee, to submit a grant
application to the Federal Transit Administration (FTA) for a Federal Jobs Access grant of$80,000 to
provide 50'% of the purchase price of eight shuttle vans to be operated by CalWORKs participants
seeking self--sufficiency. The shuttle service, which would transport children of working parents to and
from home and school and child care, is part of a three-part County strategy funded by a two-year
$784,031 grant from the Governor's 15% Special Projects Fund to provide CalWORKs participants with
job opportunities and work-related transportation services.
FISCAL:
No County cost. One hundred percent claimable to CalWORKs funding.
CHILDREN'S EAP'ACT STATEWNT:
This action will support Outcome #1, "Children Ready for and Succeeding in School", by meeting needs
to transport children between home, school, and child care. It will support Outcome ##3, „Families that
are Economically Self-Sufficient" by developing jobs and by adding services that facilitate job
commutes.
BACKGROUND:
The Governor's 15% Special Projects Fund grant provides for two years of leased cost of the shuttle
vans but does not provide for the cost of van purchase. As the grant funded program is intended to
promote entrepreneurship of CalWORKs participants, the program will be strengthened if the vans are
purchased and participants are given an opportunity to own them.

SUBJECT: AUTHORIZE SBDC GRANT MODIFICATION WITH THE STATE OF CALIFORNIA
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGORUND AND JUSTIFICATION
1. RECOMMENDED ACTION:
Authorize the Executive Director, Contra Costa County Private Industry Council, to execute
on behalf of the County an amendment to the Small Business Development Center grant to
increase the grant award by up to $30,000 from the current $300,000 level effective January
1, 1999, and further authorize the Executive Director, Contra Costa County Private Industry
Council, to execute additional non substantive amendments as may be initiated by the
California Trade and Commerce Agency.
II. FINANCIAL IMPACT:
None; 100% federal and state monies.
111. CHILDREN IMPACT STATEMENT:
None.
IV. REASONS FOR RECOMMENDATION:
The California Trade and Commerce Agency has requested that the Contra Costa SBDC
temporarily outstation consultants and clerical support in the San Francisco office of the
federal Small Business Administration to provide SBDC consultant services pending the
reorganization of the regular San Francisco SBDC which has ceased operation. This will
provide the County SBDC with additional exposure and understanding of the programs and
operation of the Regional SBA office and temporary entree to many of the businesses that are
headquartered in San Francisco.
