RECOMMENDATION(S) :
Approve and authorize the Health Services Director, cr his designee, (Frank
Puglisi, Jr. ) to execute on behalf of the County, 'Contract Amendment
Agreement #26-955-1, with Surya Kumar, M.D. (Specialty: General/
Vascular/Thoracic Surgery) , effective December 18 , 1998, to amend Contract
26-955, to extend the term of the Contract from January 31, 1999 through
December 31, 1999, and to modify the Fee Schedule set forth in the Additional
Provisions, to read as follows:
a. 950 per month, for fifteen (15) days of on-call Vascular Surgery coverage,
consultation, medical and/or surgical procedures, NOT TO EXCEED a total of $11,400; and
b. 390 per weekend, when requested by County to provide on-call General Surgery coverage,
in addition to the on-call Vascular Surgery coverage; and
C. jjA0 per occurrence, when Contractor is required to come to the Hospital during on-call
hours to perform a surgical procedure;
d. 125 per weekday when requested by County to provide on-call Vascular or General
surgery coverage more than fifteen (15) days per month; and.
e. 59.95 per hour for regularly scheduled Vascular Clinic or General Surgery Clinic
coverage, as requested by County.
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients and/or
third party payers will be billed for services .
BACKGROUND TREASON(S) FOR RECOMMENDATION(S) :
October 20 , 1998, the Board of Supervisors approved Contract ##26-955 with
Surya Kumar, M.D. , for the period from September 1, 1998 through January 31,
1999, to provide Vascular Surgery services for patients at Centra Costa
Regional Medical Center.
The need for surgery and on-call coverage has increased, and the Department
has been unable to meet these demands for services with the current on-call
staff . Dr. Kumar has agreed to provide additional Vascular and General
surgery, including weekday and weekend on-call coverage, in addition to his
current on-call services .
Approval of Contract Amendment Agreement #26-955-1 will allow the Contractor
to provide additional services and extend the term of the Contract through
December 31, 1999 . >X�'

BACRGItOMIREASON(S) FOR RECOMMENDATIONW :
On July 14, 1995, the Board of Supervisors approved Contract
#22-354-12 (as amended by Contract Amendment Agreement #22-354-13)
with Manned Parenthood: Shasta Diablo for the period from July 1,
1995 through June 30, 1999, to provide HIV/AIDS education,
prevention, and testing outreach services to women at increased risk
for HIV infection.
.Approval of Contract Amendment Agreement 22-354-14 will allow the
Contractor to provide additional services to support the Youth HIV
Prevention Social. Marketing Campaign through June 30, 1999.

for the period from July 1., 1998 through June 30, 1999, in the amount of $780,370, for
provision of intensive day treatment and medication support services for seriously
emotionally disturbed children at Oakgrove Crisis Residential/Day Treatment Programs.

CHILDREN°S IMPACT SWZMNT:
This Mental Health Services Contract will support the following Board of Supervisors`
community outcomes: "Children Ready For and Succeeding in School" "Families that are
Safe, Stable, and Nurturing"; and -Communities that are Safe and Provide a High Quality
of Life for Children and Families". Expected program outcomes are: Increase and
maintain school attendance as measured by school records; Increase in positive social
and emotional development as measured by the Child. and Adolescent Functional Assessment
Scale (CAFAS) Increase in family satisfaction as measured by the Parent Satisfaction
Survey; Decrease in use of acute care systems; and To impact placement at discharge to
a lower level of care.
BACKGROUNDJFJASON $) FOR RECOMMENIDATION(S) :
On June 24, 1997 the Board of Supervisors approved Contract #24-778-4 (as amended by
Administrative Amendment Agreement #24-778-5) with FamiliesFirst, Inc. , for the period
from. July 1, 1997 through June 30, 1998 (which included provisions for a six-month
automatic extension through December 31, 1998) for the provision of intensive day
treatment and medication support services for seriously emotionally disturbed children
at Oakgrove Crisis Residential/Day 'Treatment Programs.
Novation Contract #24-778-6 replaces the six-month automatic extension tinder the prior
Contract and allows the Contractor to continue providing services through June 30, 1999.
