RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his
designee (Mist Ca.mhi) , to execute on behalf of the County,
Contract #27-221-2 with i tong-Vi Ta, M.D. , Inc . , for the period.
from. January ", 1999 through December 31, 1999, for the provision
of professional mental health services for Conga Costa Health
Plan mer�.bers, to be paid as follows :
(1) 00 per initia, evaluation: session; and
(2) 70.00_-- for each additional therapy session.
FISCAL IMPACT:
This Contrast is funded by Contra Costa Health Plan member
premi-:ems . Costs depend upon utilization. As appropriate,
patients and/or third party payers will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Contra Costa. Health Plan (Health Plan) has an obligation to
provide professional psychiatric services for Health Plan menbers
with mental health therapy services as a covered. benefit . This
population includes Medi-Cay, medicare and Corr-nercial merrbers
enrolled in the Health Plan.
On March 3 , 1998, the Board of Supervisors approved Contract 427-
221-1 with Tuong-Vi Ta, M.D. , Inc . , for the period from January
1, 1998 through December 31, 199€3, for the provision of
psychiatric services .
Approval, of Contract #27-221-2 will allow the Contractor to
continue providing mental health services to Health Plan members
through December 31., 1999 .

SPECIFIC REQUEST(S)€R RECOMMENDATION(S)S BACKGROUND AND JUSTIFICATION
R.ECONIMENDED ACTION.
Approve and authorize the Health Services Director, or his designee
(Milt Camhi) , to execute on behalf of the County, Contract #27-218-2
W California Psychiatric Medical Croup, for the period from January
1, 1999 through December 31, 1999, for the provision of professional
mental health services for the Contra Costa Health Plan, at the
following rates :
100 . 00 per initial evaluation session; and
70 .00 for each additional therapy session.
FISCAL IMFACT:

Costs depend upon utilization, As appropriate, patients and/or third
partypayors will be billed for services.
BACKGROUND REASON(S) FOR RECOMMENDATION(S) :
The Contra. Costa Health Plan (CCHP) has an obligation to provide
professional psychiatric services for CCHP members with :Mental health
therapy services as a covered benefit . This population includes Medi--
Cal, Medicare and Commercial members enrolled in the Health Plan.
On September 15, 1998, the Board of Supervisors approved Contract #27-
21.8--1 with California Psychiatric Medica? Group, for t<he period from
U:anuary 1, 1998 through December 31, "998, for the provision of
psychiatric services .
Approval of Contract #27-2182 will allow the Contractor to continue
providing services through December 31, 1999 .

RECOMMENDATION W :
Approve and authorize the Health Services Director, or his
designee (Milt Camhi) , to execute on behalf of the County,
Contract #27-291-2 with Bay Area Retina Associates, for the period
from February 1, 1995 through Janizary 3111 , 2000, to be paid in
accordance with the rates provided in the Medi-Cal Schedule of
Maximum Allowances in effect on the date professional heath care
services are rendered to Contra Costa Health Plan Members .

On January 27, 1998, the Board of Supervisors approved Contract
#27-291-1 with Bay Area Retina Associates, for the period from
February 1, 1.598 through January 31, 1999, for the prove.sion of
professional health care services for the Contra Costa health Plan
(Health Plan) .
The Health Plan has an obligation to provide certain specialized
professional health care services for its Members under the terms
of their Individual and Group Health Plan membership contracts
with the County.
Approval of this Contract #27-291-2 will allow the Contractor to
continue to provide professional health care services to Health
Plan members through Janizary 31, 2000 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
#22-639-2 with New Connections, for the period from 'March 1, 1999
through February 29, 2000, in the amount of $126, 000, to provide
Hl-V/AIDS case management services to people with. HIV disease in
Contra Costa County.
FINANCIAL IMPACT:
"his Contract is fu-nded by the State (Federal funds) t hrouch the
Ryan Waite CARE Act . No County funds are required.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
On February 24 , 1998, the Board of Supervisors approved Contract.
#22-639-1 with New Connections for tr®e period from March 1, 1998
through February 28 , 1999 for provision of HIV/AIDS case management
services, including psychosocial assessments, coordination and
referrals, and education services.
Approval of Contract ##22-639-2 will allow Contractor to continue
providing services through February 29, 2000 .

Approve and Authorize the Health Services Director, or his designee (Chuck Deutschman)
to execute on behalf of the County, Novation Contract #24-51.3-17 with New Connections,
in the amount of $199,029, for the period from July 1, 1998 through June 30, 1999, for
provision of drug abuse prevention, intervention and treatment services. This Contract
includes a six-month automatic extension through December 31, 1999 in the amount of
$99,515.
FISCAL 1MPACj:
This Contract is included in the Health Services .Department budget, to be funded as
follows:
$124,973 Federal Funds
33,971 State Funds
40.085 Required County Match and Additional County Funds
$199,029 Total
CHILDREN'S jXFACT STATEMENT:
This Alcohol and Drug Abuse prevention and treatment program supports the Board of
Supervisors, "Families that are Safe, Stable, and Nurturing" and "Communities that are
Safe and Provide a High Quality of Life for Children and Families community outcomes
by providing individual, group, and family counseling; substance abuse education;
rehabilitation support services; and substance abuse prevention services. Expected
outcomes include increased knowledge about the impact of addiction; decreased use of
alcohol, tobacco and other drugs; increased use of community-based resources; and
increased school and community support for youth and parents in recovery.
BACKGROUNDTREASON(S) FOR RECOMMENDATlONtS) :
Or. December 16, 1337, the Board of Supervisors approved Novation Contract #24-513-15 (as
amended by Administrative Amendment Agreement #24-513-16) with New Connections for the
period froth. July 1, 1997 through ,lune 30, 1998 (whish included provisions for a six-
month automatic extension through December 31, 1998) for the provision of substance
abuse prevention, intervention and treatment services.
Novation Contract #24-513-17 replaces the six-month automatic extension under the prior
contract, and allows the Contractor to continue providing services through June 30,
1999.
