Approval of Contract #27-115-4 with Pauline Jones
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-115-4 with Pauline
Jones, for the period from January 1, 2000 through December 31, 2000, in
the amount of $88, 7u1, for consultation and technical assistance to the
Contra Costa Health Plan (Health Plan) with regard to the Psychiatric
Services, Advice Nurse and Care Coordination Programs .
FISCAL IMPACT:
This Contract is funded by Health Plan member premiums .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On January 5, 1999, the Board of Supervisors approved Contract #27-115-3 ,
with Pauline Jones, for the period from January 1, 1999 through December
31, 1999, for the provision of consultation and technical assistance to
Health Plan with regard to the Psychiatric Services, Advice Nurse and Care
Coordination Programs, including but not limited to, selection of staff,
development of training programs, and supervision of staff .
Approval of Contract #27-115-4 will allow the Contractor to continue
providing services to the Contra Costa Health Plan through December 31,
2000 .

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract
#26-945-2 with Rex Moulton-Barrett, M.D. (specialty: Hand and
Plastic Surgery) for the period from November 1, 1999 through
October 31, 2002 , to be paid in accordance with the attached fee
schedule .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and health
centers .
On September 15, 1998, the Board of Supervisors approved Contract
#26-945-1 with Rex Moulton-Barrett, M.D. , for the period from
October 1, 1998 through September 30, 1999, for the provisions of
Hand and Plastic Surgery services .
Under Contract #26-945-2, Dr. Rex Moulton-Barrett will provide Hand
and Plastic Surgery services for Contra Costa Regional Medical
Center through October 31, 2002 .

Fee Schedule
a. For consultation, training, medical and/or surgical procedures,
Contractor shall be paid as follows :
(1) $15,417 . 00 per month, for the period from November 1, 1999
through October 31, 2000, not to exceed a total of
$185, 004; plus
(2) $15, 879 . 00 per month, for the period from November 1, 2000
through October 31, 2001, not to exceed a total of
$190, 548; Plus
(3) $16,356 .00 per month, for the period from November 1, 2001
through October 31, 2002, not to exceed a total of
$196,272 ; and
b. $ 1,200 per month for on-call coverage, consisting of no more
than every other night and no more than sixteen (16) nights per
month. In the event that on-call coverage exceeds sixteen (16)
nights in a month, County shall pay Contractor $65 . 00 for each
night in excess of sixteen (16) not to exceed an annual total
of $14,400; and
C . $ 450 . 00 per occurrence when Contractor is required to come to
the Hospital during on-call hours for a surgical procedure .
In the event that Contractor works less than a full month at Contra
Costa Regional Medical Center and Contra Costa Health Centers,
County shall prorate payments to Contractor for that month; plus

Approval of Contract Amendment Agreement #23-265-1 with
Dag H. Richards
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-265-1 with
Dag H. Richards, effective November 21, 1999, to amend Contract #23-
265, to increase the Contract payment limit by $37, 400, from $25, 000
to a new total of $62 , 400 .
FINANCIAL IMPACT:
Funding for this Contract is included in the Department' s Enterprise
I budget .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
In September, 1999, the County Administrator approved and the
Purchasing Services Manager executed Contract #23-265 with Dag H.
Richards, for the period from September 1, 1999 through August 31,
2000, to provide consultation and technical assistance to the
Department with regard to computer systems administration and
networking support .
Approval of Contract Amendment Agreement #23-265-1 will allow the
Contractor to provide additional hours of service through August 31,
2000 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-266-1 with
Smartsource, Inc . , effective November 21, 1999, to amend Contract
#23-266, to increase the Contract payment limit by $490, 000, from
$25, 000 to a new total of $515, 000 .

programmers and technicians .
Approval of Contract Amendment Agreement #23-266-1 will allow the
Contractor to provide additional hours of service through September
30, 2000 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract
#26-914-7 with Stephen Weinstein, M.D. (Specialty: OB/GYN) , for
the period from January 1, 2000 through December 31, 2002 , to be
paid as follows :
a. $59 .00 per hour, for clinic coverage, consultation and
teaching, and any medical procedures that are currently
performed by Contra Costa Regional Medical and Contra Costa
Health Centers Family Practitioners with privileges in the
Department of Obstetrics/Gynecology; and
b. For medical procedures not specified above, Contractor will be
paid the actual Medi-Cal rates for said medical procedures.
FISCAL IMPACT:

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On December 15, 1998, the Board of Supervisors approved Contract
#26-914-6 with Stephen Weinstein, M.D. , for the provision of
Obstetric/Gynecology services for Contra Costa Regional Medical
Center and Contra Costa Health Centers, for the period from January
1, 1999 through December 31, 1999 .
Approval of Contract #26-914-71 will allow Dr. Weinstein to continue
providing services through December 31, 2002 .
