RECOMMENDATION:
Authorize the Health Services Director or his designee, "Wendel Brunner, M.D., to hold harmless and indemnify
Easter Hill United Methodist Church for the use of their facilities in Richmond, CA on November 13, 1998 for a
Picture This slide show event to be conducted by the Public Health Division, Family, Maternal, and Child Health
Program.

RECOMMENDATION
The Public Health Division of the County Health Services Department will hold a Picture This slide show event at
the Easter Hill United Methodist Church, 3911 Cutting Boulevard, Richmond CA 94804. This event is in
correlation with the special community based project which provides policy makers and Public Health professionals
with a snapshot of community health in Contra Costa through the eyes of the community residents.

Rental Time Limit
a. The maximum rental period is five(5)hours daily;
b. If the maximum time limit is exceeded the cost will be S25.00 per hour for each hour or fraction
thereof;
C. Total rental fee must be paid two weeks prior to the event.

The serving or consumption of alcoholic beverages on church property is prohibited;
Picture This is an innovative photography project that has been designed to provide
policy makers and public,health folks with a snapshot of community health in Contra
Costa through the eyes of community residents. This special community based project
involves community residents by giving them cameras and having them take'photographs
of people,places, things, and events in their communities that tell a story about the health
of the community. The photographs that community residents have taken offer rich,
vivid and important insights into the health concerns of residents in the county. The slide
show presentation will consist of photographs taken by community residents in West
County and the stories that they told us about these photographs.

to original state at the and of the rental period prior to the turn over of
subject facilities to the designated church representative;
b. All refuse must be bagged and placed in the refuse bin located adjacent to the
parking lot`.
c. The kitchen facility including range and oven units must be cleaned and inspected
by a designated church representative prior to the turnover of the kitchen facility;

Church is prohibited. Renters must supply their own dishware, utensils and cookinaware.

The organization or individuals renting faci t es be onging to Easter Hill United
Methodist Church agree to hold Easter Hill United Methodist Church and its membership
harmless of any and all liability emanating from the rental of Easter Hill United
Methodist Church properties and facilities.

Failure to comply with the above rules and regulations will result in the immediate
sermination of the rental agreement and denial of future rental opportunity,
