Spooner, Contracts Administrator CWlC11` DATE: June 5, 1986 (� `,'" "1 " SUBJECT: Approval of Standard Agreement (Amendment) 429-322-1 with the v State Department of Health Services (State #85-86583-A1) SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Standard Agreement (Amendment) , 629-322-1 (State #85-86583-A1) effective April 1, 1986 with the State Department , of Health Services to amend Standard Agreement #29-322 (effective July 1, 1,985 through June 30, 1986) for small water systems evaluation with a $5,670 increase in the .contract payment limit (from $56,925 to $62,595) and extending the contract term through November 1, 1986. II . FINANCIAL IMPACT: Approval of this amendment by the State will result in increased State funding of $5,670, from $56,925 to a new total payment limit of $62,595, for reimbursement of County costs under this project. No County funding is required. III . REASONS FOR RECOMMENDATIONS/BACKGROUND: On February 11, 1986, the Board approved Contract #29-322 with the State Department of Health Services for evaluation of small water systems in Contra Costa County. Assembly Bill 1803 mandated the local Health Officer to evaluate all small public water systems for contamination of groundwater sources by organic chemicals. The purpose of Standard Agreement - (Amendment) #29-322-1 is to increase the contract payment limit to cover complete reimbursement of County costs for this project, to extend the contract through November 1, 1986, and to extend the date for filing the final report with the State from September 30, 1986 to February 1, 1987. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 1, 1981 (Guidelines for contract preparation and processing, Health Services Department) . The Board Chairman should sign eight copies of the contract, seven of which should then be returned to the Contracts and Grants Unit for submission to the State Department of Health Services .
