APPROVE and AUTHORIZE Director of Social Service, or designee, to issue a Request for Proposal #1050
for transportation services for CaIWORKs participants.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOWdFNll►ED ACTION:
APPROVE AND AUTHORIZE the Director of Social Service, or designee, to issue a Request for
Proposal for transportation services worth approximately $592,855 to meet needs of CalWORKs
participants. Services, proposed in Contra Costa County's application to the Governor's 15% Special
Project fund, will create jobs in transportation and trip planning for CaIWORKs participants and will
meet transportation needs related to child care for children of participants. The grant period is from
February, 1999 through December, 2000.
FISCAL:
No County dollars. Funds for services are awarded to Contra Costa County from the Governor's 15%
Special Projects fund (State and federal dollars).
CHILDREN'S EWP'ACT STATEMENT:
This action will support Outcome #l, "Children Ready for and Succeeding in School", by meeting needs
to transport children between home, school, and child care. It will support Outcome #3, "Families that
are Economically Self-Sufficient" by developing jobs and by adding services that facilitate job
commutes.
BACKGROUND:
The Department of Social Service is fiduciary agent for a Governor's 15% Special Projects grant for a
proposal of the Contra Costa Transportation Alliance, a collaboration of County departments,
community organizations,and transportation providers. The proposal supports transportation services'
and jobs development in transportation services that benefit CalWORKs participants seeking self-
sufficiency.

RECOMMENDED ACTION
Approve and authorize the Health Services Director, or his designee
(Milt Camh'i) , to execute on behalf of the County, Contract #27-356-1
with Robert Freinkel, M.D. , Inc. , for the period from December 1, 1998
through November 30, 1999, for the provision of professional health
care services for the Contra Costa Health Plan., to be paid in
accordance with the rates provided in the Medi-Cal Schedule of Maximum
Allowances in effect on the date services are rendered',
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums.
Costs depend upon utilization. As appropriate, patients and/or third
party payors will be billed for services.
BACKGROUND/REASON'(S) FOR RECONNENDATION(S) :
On January 6, 1998, the Board of Supervisors approved Contract #27-356
with Robert Freinkel, M.D. , Inc. , for the period From December 1, 1997`
through November' 30, 1998, to provide professional health care services'
for Contra Costa Health Plan Members.
The Health Plan is also required under the terms of its Local '
Initiative contract with the State, to contract with community'
physicians and other providers, called "Safety Net" and "Traditional"
Providers, for the provision of medical care to Medi-Cal recipients .
This Contract is necessary to ,meet State mandates to expand the number '
of community providers for the Local Initiative, along with a recent '
Department of Corporations audit finding that requires formal contracts '
with low volume providers.
Approval of this Contract will allow the Contractor to continue '
providing services through November 30, 1999 .

Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Novation Contract
#24-933-2 with Crestwood Behavioral Health, Inc. , for the period from
July 1, °1998 throughJune 34, 1999, in the amount of $2,376,096, for
admission of, and treatment for, individuals who are seriously and
persistently mentally ill and in need of subacute skilled nursing care in
a facility known as an Institution for the Mentally Diseased (IMD') . This
Contract'' includes a six-month automatic extension through , December 31,
1999 in the amount of $1,188,048.

the funding source is County/Realignment.

On March 10, 1998, the Board of supervisors approved Contract #24-933 (as
amended by Contract Amendment Agreement #2'4-933--1) ' with Crestwood
Behavioral Health, .Inc. for the provision of admission and intensive day
treatment of County'-referred mentally ill persons who are in need of
sub-acute skilled nursing care in an IMD, for the period from January 1,
1998 through June 30, 1998, which included a six-month automatic
extension through December 31, 1998.
Approval of Novation Contract #24-933--2 will replace the six-month
automatic extension under the prior agreement and allow the. Contractor to
continue providing services through June 30, 1999.

124--924-2 with Jeremy Luna{-Gordinier, in the amount of $23,600,0, for
the period from November 1 1998 through October 31, 1999, to
provide consultation and technical assistance to the Department with
regard to the Duel Diagnosis Project.

This Contract is funded by 100% Federal (SA RSA) funds.
Approval of this Contract will result in a cumulative twelve-month
total in excess of $25,€00, and therefore, Board of Supervisors
approval is 'required.

In February 1998, the County Administrator approved and the
Purchasing services Manager executed Contract #24-924 with Jeremy
Lune--Gordinier, for the ;period from December 1, 1997 through April.
30, 1998, for the provision of consultation and technical assistance
with regard to the Dual Diagnosis Project. Subsequent Contract
Amendment Agreement #24-924--1 extended the term of the agreement
through October 31, 1998.
Approval of Contract #24-924-2 will allow the Contractor to continue
providing services through October 31, 1999.

Amendment Agreement #26-260-4, effective November 1, 1998, to amend
Contract #26-260- , with Preferred Staffing Incorporated, to
increase the Contract Payment Limit by $125, 000 from $78, 000 to a
new Contract Payment Limit of $200, 000.

prise I budget for FY 1998-99 .

On September 1, 1998, the Board of Supervisors approved Contract
#26-260-3 with Preferred Staffing, Inc. for the period from August
1, 1998 through July 31, 1999, to provide nursing registry services
for Contra Costa Regional Medical. Center.
At the time Contract #26-260-3 was negotiated, the payment limit was
based upon targeted levels of utilization. However,' the utilization
of services has exceeded the Department ' s projections.'
Approval of Contract Amendment Agreement ##26-260-4 will allow the
Contractor to continue providing nursing registry services through
July 31, 1999 .
