III. BACKGROUND
Senior Outreach Services (formerly known as Family and Community Services)has been
providing services to the 60+population in Contra Costa County for several years. These
services include Case Management, Home Visiting, Home Chore and In Horne Services for
the Frail Elderly Case Management Clients. The Director, Office on Aging is
recommending authorization to execute the Contract Extensions as specified above.

SUBJECT: Approval of Contract #22-379-5 with Community Alert Network
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
RECOMEND-ED ACTION:
Approve and authorize the Health Services Director, to execute on
behalf of the County, Contract #22-379-5 with Community Alert
Network, Incorporated, for the period from January 1, 1999 through
December 31, 2004, in the amount of $329, 315, for the provision of
emergency community notification services.
FINANCIAL IMPACT:
This Contract is funded in the Health Services Department ' s
Budget, and the cost of the service is offset by the fees County
charges to businesses for handling of hazardous materials .
REASONS FOR
Since 1991, the Contractor has coordinated and provided services
for an Emergency Community Notification Program within Contra
Costa County to provide large-scale telephone communications to
residents and other identified contacts in case of an emergency.
Approval of #22-379-5 will allow the Contractor to Continue to
provide these services through December 31, 2004 .

RECOMMIATION(S) :
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-222-2 with
National Institute For Chemical Studies, effective November 1,
1998, to amend Contract #23-222-1, to increase the Contract
payment limit by $5, 800, from $20, 000 to a new total Payment Limit
of $25, 800 and to extend the term of the agreement through
December 31, 1998 .
FISCAL IMPACT:
This Contract is 100-15 funded by County and Industry funds .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S)
On October 6, 1998, the Board of Supervisors approved Contract
#23-222-1 with National Institute For Chemical Studies, for the
period from September 16, 1998 through November 30, 1998, for the
provision of consultation and technical assistance to the Health
Services Director with regard to the evaluation of the Industrial
Safety Ordinance.
Approval of Contract Amendment Agreement #23-222-2 will allow the
Contractor to provide additional services and extend. the term of
the agreement through December 31, 1998 .

Approve and authorize the Health Services Director, or his designee,
(Frank' Puglisi, Jr. ) to execute on behalf of the County, Contract
Amendment Agreement #26-830-21, effective September 1, 1998, to
amend Contract #26-830-20, (effective May 1, 1998 through April 30
1999) , with Paul Reif, M.D. (Specialty: Gastroenterology) , to modify
the Contract Payment Provisions to allow the Contractor to be
reimbursed for education expenses in an amount NOT TO EXCEED $2,500 .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate',
patients and/or third party payors will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATIONS) :
On May 5, 1998 , the Board of Supervisors approved Contract #26-830-
20 with Paul Reif, for the period from May 1, 1998 through April 30,
1999, two provide gastroenterology services for Contra Costa Regional
Medical Center and Contra Health Centers .
It was the intent of the Department to reimburse Contractor for
continuous education allowing Contractor to receive additional
training in the standards of care and technology in the field of
Gastroenterology. However, due to a mutual mistake of, ,the parties
the Contract did not accurately reflect the oral agreement between
the parties, which was that the Contractor would be reimburse an
amount not to exceed $2,500 for education expenses'.
Approval of this Contract Amendment Agreement #26-830-21 will reform
the Contract, to reflect the intent of the parties, allowing Dr.
Reif to be reimbursed for education expenses through. April 30, 1999 .

#26-184-10 with Unilab Corporation, for the period from November 1,

outside clinical laboratory services for Contra Costa Regional
Medical Center and Health Centers .

This Contract is included in the Department ' s Enterprise I Budget .
As appropriate, patients and third-party payors will be billed for
services.
BACKGROUM/REASON(S) FOR RECOMMENDATIONW :
On January 20, 1998, the Board of Supervisors approved Contract
#26-184-9 with Unilab Corporation, to provide outside clinical
laboratory tests, for the period from November 1, 1997 through
October 31, 1998 . These tests require equipment not available at
Contra Costa Regional Medical Center' s laboratory and are rarely
requested laboratory tests . Because of the large number and
diversity of laboratory tests used by the medical community to make
diagnoses, most hospitals have to utilize outside laboratories as it
is too cost-prohibitive to provide all tests on site. Contra Costa
County has used the services of outside laboratories for over
twenty-five years.
Approval of Contract #26-184-10 will allow the Contractor' s services
to continue through October 31, 1999 .
