HomeMy WebLinkAboutMINUTES - 11041997 - C104 TO: BOARD OF SUPERVISORS FROM: PHIL BATCHELOR,COUNTY ADMINISTRATOR DATE: SUBJECT: AUTHORIZE THE PURSUIT OF POTENTIAL CHANGE ORDERS TO THE CONTRACT FOR BID PACKAGE NO.3-CONSTRUCTION OF NEW HOSPITAL,2500 ALHAMBRA AVENUE,MARTINEZ(WH580B) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION L RECOMMENDATION AUTHORIZE the issuance of Change Orders to the Contract dated February 7, 1995 issued to Centex Golden Construction Co. for the Construction of a New Hospital at 2500 Alhambra Avenue, Martinez, and AUTHORIZE the County Administrator or his designee to execute the Change Orders for 14 potential changes described in Section III below. II. FINANCIAL IMPACT The total NTE amount is $1,213,000. Should these changes be implemented, funds in the capital account established for the project would be sufficient to cover the amount encumbered by this contract, from bond proceeds specified to the project. Bond financing for the total project cost was secured in May 1992 in accordance with the Board of Supervisors' direction. REASONS FOR RECOMMENDATIONS The following items are those current potential changes, which represent exposure greater than $25,000. Scope and final costs are currently under negotiation. Authorization is requested to finalize negotiations and issue change orders for this work,within the corresponding Not-To-Exceed(NTE)amount. Proposed Change Order No 591-Revisions to the supply entrance. NTE $70,000 The contract documents required that E ward be remodeled and a new ramp added alongside the old building to provide egress from the new hospital. Due to various issues, including the electrical feeder routing and asbestos abatement, the Owner decided that a portion of this work should be left in place. As a result, an alternate means of exiting must be provided. The revised exiting provisions include a new stair and the associated civil and landscape changes. Proposed Change Order No. 637-Smoke compartment modifications NTE $35,000 To compensate for the loss of smoke compartments,as a result of previous I-3 occupancy changes(the addition of secure psych units), OSHPD mandated that an alternate smoke compartment be added. The scope includes addition of cross-corridor doors and fire/smoke dampers. Proposed Change Order No. 662-Light pole base foundation interference NTE $30,000 The contract calls for the site light poles to be mounted on 24" diameter x 5'deep concrete piers. The contractor observed that the front entrance was so congested with existing utilities that damaging those utilities, while drilling a 5'deep pier, would occur. As a result, the architect provided an alternate mat foundation footing for these light poles at various congested utility areas on the site. The new detail increased the material and labor quantities significantly. Proposed Change Order No. 665-Signage changes NTE $43,000 Just prior to the signage production, the Owner revised the room-numbering scenario and changed the designation for several rooms. In addition, the name of the facility was changed from Merrithew Memorial Hospital to Contra Costa Regional Medical Center. Proposed Change Order No. 676-Linen chute collection room revisions NTE $ 30,000 Code mandates that a linen chute termination room have a 2-hour rating. The OSHPD Fire Marshal noted that Linen Collection Room 1202 was designed and constructed with a 1-hour rating. Walls and ceiling will require removal and reinstallation with a 2-hour rating. In addition, a new 90-min rated door and fire damper are required. Proposed Change Order No. 691-Rated door and frame installation NTE $50,000 The OSHPD Fire Marshal life/safety inspection noted numerous non-conforming doors/frames, smoke detectors and fire dampers. As a result, approximately 15 doors/frames need to be remanufactured and numerous fire dampers have to be retrofitted. Proposed Change Order No. 695-Fire damper and wall changes to special use areas. NTE $90,000 During a pre-review of the fire alarm system by the OSHPD Fire Marshal, numerous changes were identified. These items included wall modification, fire dampers, additional strobes, pull stations and smoke detectors. Also, several field obstructions infringed on the allowable spacing of installed detectors which resulted in additional units. Proposed Change Order No.724-Door and UPC changes. NTE $30,000 Due to exiting width violations, the OSHPD Fire Marshal mandated that exterior door 3435 be removed. The rational is that no door is better than a door that violates width requirements. In addition, 28 Universal Precaution Centers (UPCs)projected more than 3" into the hallways. As this is a code violation,the UPCs were removed. Proposed Change Order No.725-Medical gas storage revisions. NTE $75,000 Per previous directives, a med gas storage area was added to room 3518A. Contract documents also depicted a med gas supply room in room 2108. These rooms did not have a dedicated exterior exhaust system per NFPA 4-3.1.2. As a result, existing exhaust ductwork was blanked off and new dedicated exhaust fans and ductwork, which vent to the exterior,were added. Page 2 of 3 Board Order to Approve Issuance of Potential Change Orders for Merrithew Memorial Hospital Replacement Project Bid Package No. 3 Proposed Change Order No.735-Psych Emergency Services(PES)Unit changes NTE $ 125,000 After Owner review of the 3`d floor PES Unit with representatives from Licensing, numerous changes were requested. Changes include adding a patio security cover and revising security egress controls, hardware, nurse station,and bathroom accessories. Proposed Change Order No.789-Fin tube enclosure NTE $ 30,000 The Contractor contends that the fin tube enclosures, which cover the radiant heating elements, were not detailed adequately. As a result, they had to make a custom fin tube enclosure that circled the interior of the front window wall. The final scope and exposure, related to this change, is still under review. Proposed Change Order No.790-Revisions to the film processing area NTE $30,000 Under previous directives, changes have been made to the film processors and printers in the Radiology Department. Scope includes additional architectural, mechanical and electrical revisions. Specific items include: sleeve patching,door revisions,and duct/drain deletions and rerouting. Proposed Change Order No.791-Extended general conditions NTE $ 334,000 Although the contract completion date was September 30, 1997, numerous Owner changes will keep the contractor on site until December 31, 1997. Based on this and increased staffing to manage changes,the Contractor is requesting compensation for the extended job site overhead and manpower. Proposed Change Order No.768-Psychiatric and Geriatric Unit changes. NTE $250,000 To assure patient safety in the Psychiatric and Geriatric Units, Licensing directed that existing shower heads and lavatory fixtures be removed and replaced with security fixtures. Total scope includes: remove 31 shower heads and lavatory fixtures and replace with security fixtures, and patch and repair damage caused by this change out. In addition, several security system changes were mandated, various doors were required to be locked, some toilet accessories are being removed, and bedpan washers are being removed and capped.
