RECOMMENDATION APPROVE the issuance of Change Order No. 133 to the Contract dated February 7, 1995 issued to Centex Golden Construction Co. for the Construction of a New Hospital at 2500 Alhambra Avenue, Martinez, and AUTHORIZE the County Administrator or his designee to execute the Change Order. II. FINANCIAL IMPACT Change Order No. 133 totals$150,116.00. Funds in the capital account established for the project are sufficient to cover the amount encumbered by this contract, from bond proceeds specified to the project. Bond financing for the total project cost was secured in May, 1992 in accordance with the Board of Supervisors' direction. REASONS FOR RECOMMENDATIONS Change Order No. 133 will provide for the following: Proposed Change Order No.536-Smoke/Heat Detectors $31,886.00 OSHPD review of the fire alarm shop drawings noted several deficiencies. Deficiencies included insufficient and incorrect detector types. Proposed Change Order No. 627-Paving at B Street and Street 5 73,315.00 Several areas surrounding the site were not scheduled to receive paving. In order to leverage existing paving efforts and provide consistency, direction was given to also include these areas in the new paving program. The scope included removing/replacing asphalt and pavement striping. Proposed Change Order No.485.2-Steel, Studs @ Louvers $44,915.00 Contract documents call for acoustical louvers at the air intake and exhaust plenums around the exterior of the penthouse. Contract specifications called for a specific louver manufacturer. Based on this manufacturer's design criteria, no intermediate mullion support was provided. During the course of construction,the contractor substituted louver manufacturers. The substituted manufacturer required the addition of intermediate mullion supports. Review of the bid pricing revealed that the louvers were bid without the intermediate support,and as a result,the additional support work is value added to the project. Page 2 of 2
