APPROVAL OF LIHEAP 1996 CONTRACT AMENDMENT AGREEMENT WITH THE
CALIFORNIA DEPARTMENT OF COMMUNITY SERVICES AND DEVELOPMENT.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S)6 BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Community Services Director or designee to execute and submit to
the California Department of Community Services and Development 1996 State Standard Agreement
Amendment No. 2 (County #39-806-4, State Contract #96B-8003) to extend the contract period from
January 1, 1996 through December 31, 1996 to March 31, 1997 for the provision of low-income home
energy assistance program services.
II. FINANCIAL IMPACT:
This Contract is Federally funded through the California Department of Community Services and
Development. The maximum reimbursable amount does not increase with this proposed extension. No
County funding is required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On February 6, 1996 and July 16, 1996 the Board approved State Standard Agreement #96B-8003
(County #39-806-1) and its amendment (County #39-806-2), respectively, with the California
Department of Community Services and Development (State) for a total maximum reimbursable
amount of $229,930 in 1996 Low-Income Home Energy Assistance Program (LIHEAP) funding for
the period from January 1, 1996 through December 31, 1996 to continue home energy assistance,
energy crisis intervention, and weatherization services to County individuals and families.
In a letter dated November 22, 1996 from the Department to the State, the Department requested a
contract extension of the termination date from December 31, 1996 to June 30, 1997 in order to
complete weatherization of the projected number of homes as specified in the Agreement. The State
responded to the request by granting an extension making the termination date March 31, 1997.

I . RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Standard
Agreement (Amendment) #28-570-2 (State #94-21052 02) , effective
September 30 , 1996 , with the State Department of Health Services for
the Bicycle Head Injury Prevention Project, to decrease the contract
payment limit by $20, 000 from $369, 935 to a new total of $349, 935 .
II . FINANCIAL IMPACT:
This Amendment decreases the maximum reimbursable amount of funding
under this Agreement by $20 , 000, from $369, 935 to a new total of
$349, 935, as' a result of a reduction in federal Centers for Disease
Control (CDC) funding to the State . No County funds are required.
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
On August 8 , 1995, the Board of Supervisors approved Standard
Agreement #28-570 (as amended by Standard Amendment Agreement #28-
570-1) with the State Department of Health Services, for the period
from October 1, 1994 through September 30, 1997 for the Bicycle Head
Injury Prevention Project . The purpose of this project is to
conduct a pilot study to determine the efficacy of helmet
distribution campaigns in conjunction with , bicycle helmet
legislation as a means to reduce the instance of injury and fatality
to bicyclists due to head injury.
This Amendment is a formal method for the State to disencumber funds
to reflect the reduction in CDC monies for this project . The
decrease in funding will not significantly impact the pilot study
and there is sufficient funding to maintain the project through
September 30, 1997 .
Three sealed and certified copies of this Board Order should be
returned to the Contracts and Grants Unit for submission to the
State Department of Health Services .

II. FINANCIAL IMPACT:
Acceptance of this Award from the CJ Foundation for Sudden Infant
Death Syndrome will result in $4, 000 to support the activities of
the Department' s SIDS Program. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
This Award will fund a project to assure that health care
providers in Contra Costa County are adequately informed of the
County SIDS Program services and the "Back to Sleep"
recommendations. An informational package will be designed and
mailed to each physician, midwife, nurse practioner, registered
nurse, and licensed vocational nurse, to ensure that providers are
equipped to provide this valuable information to families and care
givers.

I. RECOMMENDED ACTION:
Accept three awards (County #29-393-4, #29-393-5 and #29-393-6) ,
in the amount of $258,780, from the State Department of Health
Services (Tuberculosis Control Branch) , for the period from July
1, 1996 through June 30, 1997, for the Tuberculosis (TB) Control
Program.
II. FINANCIAL IMPACT:
Acceptance of these Awards will result in a total of $258,780 in
State Local Assistance and CDC funds during FY 1996-97 for the
County' s TB Control Program. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Health Services Department maintains a TB Control Program
which serves all reported TB patients and their contacts in Contra
Costa County. Acceptance of these Awards will allow the
Department to: (1) continue to expand prevention and control
activities, and (2) improve completion of appropriate therapy
which is essential to decrease TB transmission, prevent the
development of drug resistance, and cure TB patients.

I . RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract
Amendment Agreement #29-611-15 with the Office of Statewide Health
Planning and Development, to amend Standard Agreement #29-611 (State
#76-57145) . This amendment, effective July 1, 1997, continues the
Family Practice Residency Program through June 30, 2000, and increases
the contract payment limit by $51, 615, for a new total of $672 , 970 .
II . FINANCIAL IMPACT:
Approval of this amendment by the State will result in increased
funding of $51, 615, for a new total contract payment limit of
$672, 970, for the Family Practice Residency Program. No County funds
are required.
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
On January 31, 1978 , the Board of Supervisors approved Contract #29-
611 with the State to implement the County' s Family Practice Residency
Program for the period from February 16 , 1977 through June 30 , 1980 .
Subsequent amendments to the contract were approved by the Board to
extend the program and continue State funding. The purpose of
Contract Amendment Agreement #29-611-15 is to continue the program
through June 30, 2000 .
Four certified and sealed copies of this Board Order should be
returned to the Contracts and Grants Unit for submission to the Office
of Statewide Health Planning and Development .

through June 30, 1997, to pay the County $18 , 000, for provision of
professional mental health intervention services for certain Special
Education students.
II. FINANCIAL IMPACT:
Approval of this Agreement will result in a total payment to the
County of $18, 000. No County match is required.
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
Under the terms of this Agreement, #29-485-2 , the County
Superintendent of Schools (Office of Education) will pay the Health
Services Department' s Mental Health Division a total of $18, 000 to
provide professional mental health intervention services to County-
designated severely emotionally disturbed Special Education
students, who are enrolled at the Office of Education's' Marchus
School and who are participating in the AB 599 Pilot Project, and
their families.

RECOMMENDATION:
Authorize the Health Services Director,or his designee, (Wendel Brunner, M.D.) ,to execute on behalf of the County,
a contract with the State Department of Health Services (DOHS), for FY 1997-2000 in an amount not to exceed
$840,000.

BACKGROUND:
Contra Costa County has the second highest incidence rate of breast cancer in California. In Contra Costa, women of
color are particularly at risk. Within California, Contra Costa County has the highest incidence of breast cancer
amongst Hispanic women, the second highest amongst African American women and the third highest rates for Asian
Pacific Islander women.
With an initial grant of$390,000 from a special state tobacco tax,the Contra Costa Health Services Department formed
a Partnership of breast cancer survivors, advocates, community based organizations, health care providers and
individuals dedicated to making the early detection of breast cancer a community wide priority.
Since May of 1995, the Contra Costa Breast Cancer Partnership has developed a network of 32 private and public
health care providers who now provide low income, uninsured and underinsured women access to free breast exams
and mammograms. Concurrently, the Partnership launched an aggressive outreach and education campaign to the
community using both tailored and broad based outreach strategies. In August 1996, the Partnership funded 11
community based organizations to conduct culturally tailored outreach to African American, Latino and Asian
communities in Richmond/San Pablo and the Pittsburg and Antioch areas. The Partnership has sponsored broad based
outreach to all women through radio and TV ads, interior bus card advertising and direct mail to remind women of the
importance of annual breast health check-ups. By February 1997, the Partnership will have screened 700 women.
The Partnership has been refunded for a total of$840,000 to continue its efforts through February 2000. Activities will
focus on priorities identified through an active community planning process. The Contra Costa Health Services
Department is one of only a few county health departments to receive these funds.
We have been verbally informed that the application for continued funding through 2/29/2000 has been approved. The
contract must be fully executed prior to March 1, 1997, however, the State did not supply the contract document in
sufficient time to process it to the Board for approval. No retroactive payments will be made for a period prior to
execution. The Board of Supervisors approved submittal of the application for this continued funding on 10/15/96.
FISCAL IMPACT:
This will be additional revenue to the County. J

Service contract with Mr. Doug Drewes is amended for an additional amount of$10,000
raising the cap to $35,000. The service contract term is extended from January 1, 1997
to June 30, 1997.
II. Financial Impact:
None. All costs are supported by fees.
III. Reasons for Recommendations and Background:
The Public Works Department has employed this consultant to provide assistance with Computer
Aided Drafting (CAD) systems operations and administration. The required work will exceed the
$25,000 payment limit on the contract so the limit needs to be increased.
IV. Consequences of Negative Action:
The required work will not be accomplished.

RECOMMENDATIONS:
Approve and authorize the Purchasing Services Officer to increase the
existing purchase order contract with Professional Process Service by
$15, 000 for a total annual contract amount of $40, 000 . The contract
covers the service of civil process for the District Attorney Family
Support Division.
FINANCIAL IMPACT:
None . There is no net county cost associated with the approval of
this request . The total cost is covered by 66% Federal Funding and
34% SEIF (Support Enforcement Incentive Funding) .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
The District Attorney' s Family Support Division must comply with
Government Code §26608 to "serve all process" . All of the out of
County service of process is being completed by a private process
service company.
As the Family Support caseload increases year to year there is a
greater demand for using the outside vendor. Professional Process
Service was chosen by the "Request for Proposal" process through the
Purchasing Department.

CONSEQUENCES OF NEGATIVE ACTION:
Failure to approve this request would hamper the District Attorney' s
Family Support Division in its attempt to comply with Government Code
§26608 to "serve all process" . Inability to serve all process could
cause Family Supports operation to establish paternity and support
orders to grind to a halt and ultimately generate a positive net
county cost for the Family Support Program.

APPROVE and AUTHORIZE the Community Services Director or designee to execute Short
Form Contract Amendment/Extension #38-327-5 with Patricia Brown, R.N., effective December
31, 1996, to extend the Contract Term through June 30, 1997 and to increase the Contract
Payment Limit by $18,550 from $12,350 to a new Total Contract Payment Limit of $30,900 for
the provision of Head Start Health Services.
II. FINANCIAL IMPACT:
No County funding is required. Funding is from the U.S. Department of Health and Human
Services Administration for Children and Families and is included in the Department's FY 1996-97
Budget.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND
In September 1995 the County Administrator approved and the Purchasing Agent executed a
contract with Patricia Brown, R.N. the Head Start Health Services Consultant for the period from

ECCDC and the Board of Trustees of Bi-Sett Corporation.
Approval of Contract #24-872 will allow Bi-sett Corporation to
continue the perinatal and residential treatment services provided
for women at the ECCDC facility, through June 30, 1997 .

II. FINANCIAL IMPACT:
This Contract is funded in the Health Services Department's
Enterprise I Budget. Under the terms of this Exchange of Use
Contract, the County's Merrithew Memorial Hospital provides
certain medical services for Veterans Administration Outpatient
Clinic (VAOPC) patients, and VAOPC provides certain medical
services for the County' s patients. Each party is paid a fee for
service in accordance with the fee schedules incorporated in the
Contract. There is no unit price increase included in this
Amendment.
The services provided for the County's patients under this
Contract are billable to patients and third party payors.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND: 11 1
On May 13 , 1993 , the Board of Supervisors approved Exchange of Use
Contract #V612P-4484 (as amended) with the U. S. Department of
Veterans Affairs for provision of certain specialized medical
services by each party through December 31, 1996.
This Amendment provides for a three-month extension of the
Exchange of Use Contract, for the services and at the rates
specified in the Contract, for the period from January 1, 1997
through March 31, 1997.
The Contract documents have always been prepared by the Veterans
Administration.

(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-428-8 with AIDS Community Network, effective
January 1, 1997, to amend Standard Contract #22-428-5 (as amended by
Contract Amendment Agreements #22-428-6 and #22-428-7) , to increase
the payment limit by $3 , 000 from $54 , 500 to a new total Contract
payment limit of $57, 500.
II. FINANCIAL IMPACT:
This Contract is funded by federal funds through the Ryan White CARE
Act Title I, Formula & Supplemental Grants, through intergovern-
mental agreements with Alameda County, who is the Grantee of these
funds. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In June 1996, the County Administrator approved, and the Purchasing
Services Manager executed Standard Contract #22-428-5 with the AIDS
Community Network, to provide Day Center support services for
persons with HIV for the period from April 1, 1996 through July 31,
1996. Subsequent Contract Amendment Agreements #22-428-6 and
#22-428-7 extended the term of the Contract through March 31, 1997 .
Approval of Contract Amendment Agreement #22-428-8 will allow the
Contractor to provide additional services through March 31, 1997.
