BACKGROUND: On July 15, 1997, the Board of Supervisors approved a report from our Committee which included the following recommendations: 1. ACCEPT the following report from the Family and Human Services Committee on the status of the Service Integration Projects. 2. DIRECT the County Administrator's staff to report to the Family and Human Services Committee during the month of December on the status of the Service Integration Projects since July. On December 8, 1997, our Committee met with the County Administrator; Sara Hoffman, Senior Deputy County Administrator; Nina Goldman, Interim Program Manager for the Service Integration sites and representatives from_ SEIU Local 535. Ms. Goldman reviewed the attached report with our Committee is some detail. Mr. Batchelor explained the purpose of the Executive Oversight Committee as a mechanism to more fully involve the department heads in the work of the Service Integration sites. We are very pleased to see the progress which has been made in tracking participant outcomes in terms of employment levels, school attendance and the success of child welfare interventions. We believe that it is about time to begin to measure the cost-effectiveness of Service Integration. Service Integration Team workers carry smaller caseloads that other workers. We need to begin to see whether these smaller case loads are resulting in overall savings to the County. We are also interested in seeing whether the increased levels of employment are actually resulting in individual's being able to better support their families. We are, therefore, asking for data on what kinds of jobs participants are getting, what levels of pay they are receiving and how long they are able to hold these jobs. In response to a question from Supervisor Gerber about future expansion plans for Service Integration, Ms. Goldman and Ms. Hoffman indicated that the identified areas which need to be served include the Monument Corridor in Concord, Pittsburg, and several sites in West County. Supervisor DeSaulnier asked for a report on the feasibility of expanding to the Monument Corridor in the next year. Introduction Contra Costa's Service Integration Teams (SlTeams), comprised of staff from the County Departments of Health Services, Probation and Social Service and the Mount Diablo Unified School District's Healthy Start Program, have been providing family-focused, strengths-based, integrated services to the communities of Bay Point and North Richmond for over three years. The Service Integration Program strives to achieve the following three goals: (1) to increase family economic self-sufficiency; (2) to improve family stability; and (3) to expand community capacity to support children and families. This report presents some of the highlights of the activities, successes and challenges of Contra Costa County's Service Integration Teams (SlTeams) since the last report to the Family and Human Services Committee of the Contra Costa County Board of Supervisors in June, 1997. In particular, the report focuses on the role of the Service Integration Program given the advent of Federal, State and County welfare reform and illustrates how the work of the SlTeams furthers welfare-to-work goals.by: (1) taking a comprehensive approach to helping low-income families transition to economic self-sufficiency and stability; and (2) working with some of the County's most at-risk communities to expand their capacity to sustain healthy children and families over the long-term. *This report also provides an update on other issues related to the Service Integration Program. II. Year-to-date Outcomes for 1997 A critical aspect of the Service Integration program is its emphasis on tracking participant outcomes. Using the outcomes tool, the SlTeams are able to judge their success based 1 on measurable improvements in the lives of children and families. Bay Point SlTeam Outcomes The Bay Point SlTeam has maintained last year's strong record of moving Bay Point residents from welfare to work. 100 AFDC recipients served by the Bay Point SlTeam obtained employment from January through September 1997. The rate of labor force participation by AFDC recipients served by the Bay Point SlTeam continues to soar. As of September 1997, 24.5 percent of the AFDC recipients served by the Bay Point SIT reported earned income. In contrast, the figures for the Antioch District Office and Contra Costa County as a whole were 21.7 percent and 21.8 percent respectively. During the past year, the share of Bay Point welfare recipients with earnings from employment has increased far more dramatically than for the Antioch District Office and the County as a whole (See attached Figure 1). During the first three months of the 1997-98 school year, the share of Bay Point youth on probation enrolled in school has remained over 94 percent. North Richmond SlTeam Outcomes The North Richmond SlTeam has significantly increased its effectiveness at getting North Richmond residents to work. Between January and September 1997, 50 AFDC recipients served by the North Richmond SlTeam obtained employment. Over the comparable period in 1996, only 22 AFDC recipients served by the North Richmond SlTeam got jobs. While labor force participation is increasing throughout the County, AFDC recipients served by the North Richmond SlTeam are moving even more rapidly into the labor force than AFDC recipients in comparable circumstances elsewhere in the County. Between September 1996 and September 1997, the share of AFDC recipients with earned incomes served by the North Richmond SlTeam rose from 13.3 percent to 21.2 percent. In contrast, during this same period, the percentage of AFDC recipient with earned incomes served by the Richmond District Office rose from 13.0 percent to 17.1 percent (See attached Figure 2). The North Richmond S/Team's child welfare strategy is resulting in avoided out-of-home placements and court involvement. Of the 50 families receiving child welfare services from the North Richmond SlTeam between July and October 1997, court involvement was necessary in only 1 case. Of 1.33 children receiving child welfare services during this period, only 3 were removed from their homes. 2 III. Service Integration and Welfare Reform This section addresses three ways in which Service Integration plays a critical role in a welfare reform environment. Time limits,work requirements and other mandates of welfare reform pose a dilemma: how to efficiently and effectively move long-term, multi-problem CaIWORKs participants toward economic self-sufficiency. SIT has been working for over three years to develop new strategies for addressing this issue. The result is a program that has blazed new paths, that complements the activities planned under welfare reform and that strives for goals that go beyond those mandated by welfare reform to increase the well-being of some of the County's most distressed communities. (1) Service Integration has pioneered and continues to pioneer the development of new service delivery processes and models as well as alternative organizational structures for serving Contra Costa Families. Activities developed by SIT that are now part of, or planned, in Contra Costa's welfare reform model include, but are not limited to: • Integration of eligibility and employment activities, including the development of self- sufficiency workshops. • Emphasis on employment and supportive services in all staff/participant contacts. • Collaboration across programs, departments, agencies and with community-based organizations. • Staff training in team building and organizational culture change. • Emphasis on outcomes over process and tracking of key outcome measures. • Focus on the whole family, not just the individual. • Strengths-based, as opposed to needs-based,assessments and intervention strategies. • Expanded staff participation in and consequent"ownership" of program processes and activities. • Community involvement in the development of services. • Engagement of program participants as "partners" in the service delivery model. • Procurement of a State and Federal waiver to increase the efficiency and effectiveness of services. 3 Service Integration continues to serve as a "laboratory" for developing and testing improved service delivery methods and organizational structures. Currently, the SITeams are in the process of refining their case management model in partnership with several community members to achieve better outcomes for families served. We hope that the resulting model will be of value countywide. Some other examples of recent SIT successes in pioneering new program initiatives include: `r In September 1997, the Service Integration Program was awarded first year funding of approximately $203,000 from the Zellerbach Family Fund to support the establishment of community-wide employment systems in Bay Point and North Richmond. (Second and third year funding, totaling $400,000, are contingent upon successful performance in Year 1). This grant not only will not only create jobs and build skills for numerous community members, but also will build the foundation for future collaboration by establishing strong partnerships across County agencies, local non-profit organizations and community residents. On September 18th, the Bay Point SlTeam co-hosted the second annual Community Employment Outreach (CEO) Job Fair at the Ambrose Community Center. An estimated 800 job-seekers and over 25 private companies and non- profit employment brokers attended the Job Fair. In January 1998, the Bay Point Jobs Center, an employment resource and skills center run by community members in partnership with the Bay Point SlTeam, will open its doors. The Jobs Center will be open to all interested Bay Point community members. Through SIT's establishment and facilitation of the North Richmond Employment Collaborative, SlTeam members worked with community members, community-, based organizations,other public programs(including the Center for Health, Healthy Neighborhoods and Partners for Health programs of the Health Services Department) and AC Transit to bring back desperately needed night bus service to the North Richmond community. November 24, 1997, marked the launching of the first North Richmond/Parchester Village Night Jobs Shuttle. This effort was widely lauded as the first example statewide of a community rising to demand and obtain public transportation services necessary to achieve the demands of welfare reform. The story was reported on. KQED radio, Channel 2 and Channel 5 news and in a number of local papers (See Attachment A). (2) By targeting those families served by multiple County programs, Service Integration complements local welfare reform activities by filling a specific set of needs not common to the majority of CalWORKs participants. Service Integration addresses the needs of families who are involved not only with cash aid and work readiness programs, but also with child welfare, mental health, probation, 4 public health and school-based programs. The integration of services allows staff from multiple agencies to develop and implement a consistent case plan across programs. For example, staff working with a parent receiving both GAIN and Probation services develop with the family a clear set of priorities that meet the mandates of both programs without conflicting requirements for the parent. By providing multi-problem families with a more intensive service strategy designed to address the complexity of their needs, the Service Integration Program fills a service gap for the district offices, which do not have the resources or the mandate to address the many needs of these cross-agency families. Examples of special services being implemented at SIT to address the needs of multi- problem communities include: Working in partnership with the Mt. Diablo Unified School District, the Bay Point SITeam's Mental Health Counselor has spearheaded Anger Management Groups targeting school-aged children in three Bay Point Elementary Schools who are exhibiting problematic behaviors in the classroom. In an innovative approach to reach elementary school children in North Richmond, Child Welfare and Mental Health SlTeam members have established an Art Therapy class that is integrated into the children's day at Verde Elementary School and allows at-risk children to develop constructive means of self-expression. (3) Service Integration embraces a broader set of goals than are required for the County's welfare reform model. Service Integration was designed for nine economically and socially distressed communities that were identified as having the highest concentration of service needs and as lacking community infrastructure. In order to provide appropriate services in these environments, an emphasis on strengthening communities as well as families is vital. This emphasis, embodied in the Service Integration Program, is resource-intensive and, therefore, is only utilized in those specific neighborhoods where it is deemed most necessary. The goals of SIT go beyond the economic self-sufficiency goals of welfare reform, because the SIT neighborhoods demonstrate particularly high levels of need. In addition to the goal of increasing the economic self-sufficiency of families, the SlTeam members strive to increase family stability through their-work with families. The holistic nature of the SIT approach is measured in a wider set of family outcomes than are mandated under welfare reform, including: increased school attendance, more children remaining safely in their homes, lower levels of probation recidivism, improvements in community safety and aesthetics, greater involvement in community activities and improved child health. 5 Examples of the Service Integration Program's participation in the broad array of issues affecting community well-being include: The Bay Point SlTeam recently entered its fourth year of successful collaboration with the Mt. Diablo Unified School District's Healthy Start Program. The participation of two Healthy Start Family Resource Specialists on the Bay Point SlTeam has allowed the program staff to successfully address a wide range of school-related issues affecting families, including behavioral problems, tardiness and absenteeism. In addition, this partnership has resulted in greater access to County Services for families involved in the school system. In one recent example, the Bay Point SlTeam has worked with staff from Foster Youth Services and Healthy Start and teachers to establish a seamless system of referring children with identified mental health problems to the SlTeam for services. This partnership has led to such a high number of referrals that the Foster Youth Services program has made available Social Work interns to meet the high demand. r In the North Richmond community, where issues of hunger and food shortages remain of great concern, the North Richmond SlTeam established a community garden in partnership with the Laotian Organizing Project of the Asian Pacific Environmental Network (APEN). Currently, approximately 40 families are farming on the previously unused land surrounding the North Richmond Family Service Center. In addition to producing healthy food for their own consumption, all of the gardeners donate approximately 10 percent of what they grow to other needy families in the community (See Attachment B). IV. Other Updates Establishment of the Service Integration Executive Oversight Committee In October 1997, an Executive Oversight Committee was established for the Service Integration Program. The Oversight Committee is comprised of. Phil Batchelor, County Administrator; John Cullen, Director, Social Service Department; Terry Starr, Chief Probation Officer, Probation Department; and William Walker, Director, Health Services Department. The Oversight Committee will meet bi-monthly to: • Provide overall policy direction for the Service Integration Program; • Resolve cross-departmental issues; • Determine how best to ensure that successful SIT strategies are institutionalized in the home departments; 6 • Explore and identify areas for demonstration of innovative service delivery models and improved organizational relationships at the SITs. SIT Participation in Labor-Management Cooperation Conference In early November 1997, representatives from the Service Integration Program and from the Contra Costa Labor Coalition participated in a symposium on Reforming Human Services Through Labor-Management Cooperation. The seven-person Contra Costa delegation spent three days at the conference in Chicago, and gave a joint labor- management presentation on the Service Integration Program. The trip was paid for in full by the Annie C. Casey Foundation. The Service Integration Program was recruited to participate in the conference based on its track record for innovation in the public sector. First Issue of SIT Insights Published The first issue of the Service Integration Program's new newsletter, SIT Insights, was produced in November 1997. The purpose of the newsletter is to share information and ideas emerging from the SITeams with staff at County departments,funders, policy makers and the families the program serves. The newsletter will be issued on a quarterly basis (see Attachment C). V. Challenges Implementation of the Bay Point Works and North Richmond Employment Collaboratives The Zellerbach Family Fund recently awarded supportfor the ambitious community-wide employment strategies developed by the Bay Point Works and North Richmond Employment Collaboratives. Two hiring panels, each comprised of representatives from the Service Integration Program and other community programs and community members, recently hired Project Managers for each initiatives. The new Project Managers will come on board in mid-December. Implementing the plans described in the Zellerbach proposals will pose an exciting challenge. Filling Service Gaps The Service Integration Program continues to strive to fill certain gaps in service, most notably substance abuse and adult mental health services. These two areas, which unfortunately are under funded by the Federal and State governments, have been identified by both the Bay Point and North Richmond communities as areas where resources are sorely needed. (Currently, there are no substance abuse or adult mental health components at SIT.) In the coming year, we will strive to procure resources to ensure that SIT provides more comprehensive services that meet these needs. 7 Continuing to be an Innovator for the Wider County System Over the past more than three years, the Service Integration Program has served an important role in Contra Costa: SIT has continually strived to put the stake out in front of other County programs, successfully developing and implementing innovative service delivery models and organizational structures for improving the well-being of children and families. Recently, with efforts such as the establishment of the SIT Executive Oversight Committee and the production of SIT Insights, the Service Integration Program has been focusing on identifying the piloting needs of County departments and strengthening the channels for communicating the lessons of Service Integration back to the wider County system.
