SUBJECT: Approval of Contract Amendment Agreement #22-522-4 with the
United Council of Spanish Speaking Organizations, Inc.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTIONS:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-522-4, with the United Council of Spanish
Speaking Organizations, Inc. , (U.C.S.S.O. ) effective September 1,
1996, to amend Contract #22-522-2 (as amended by Contract Amendment
Agreement #22-522-3) , to increase the payment limit by $12 , 000 from
$27, 000 to a new total Contract payment limit of $39, 000.
II. FINANCIAL IMPACT:
This Contract is included in the Health Services Department' s Budget
and is funded by State Ryan White Title II funds. No County funds
are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In April 1996, the County Administrator approved and the Purchasing
Services Manager executed Standard Contract #22-522-2 with
U.C.S. S.0. , for the period from April 1, 1996 through July 31, 1996,
to provide door-to-door round trip transportation for people with
HIV, to and from health care or social service .appointments. A
subsequent Contract Amendment Agreement #22-522-3 extended the term
through March 31, 1997.
Approval of Contract Amendment Agreement #22-522-4 will allow the
Contractor to provide additional services through March 31, 1997.
