SUBJECT: Approval 'of Contract Amendment Agreement #22-376-2 with
Center for Human Development
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) ac BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-376-2 with Center for Human Development,
effective October 1, 1996, to amend Contract #22-376-1, to increase
the payment limit by $15, 397 from $14, 603 to a new Contract Payment
Limit of $30, 000.
II. FINANCIAL IMPACT:
This Contract is funded 100% by State Tobacco Tax funds. No County
match is required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In August 1996, the County Administrator approved and the Purchasing
Services Manager executed Short Form Service Contract #22-376-1 with
Center for Human Development for the period from August 1, 1996
through July 31, 1997 for consultation and technical assistance to
the Department with regard to reducing the impact of tobacco
consumption in Contra Costa schools.
Approval of Contract Amendment Agreement #22-376-2 will allow the
Contractor to provide additional services through July 31, 1997 .
