MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: January 9, 1996 SUBJECT: Approval of Contract Change Order No. 3 for the San Pablo Dam Road Slide Repair at Station 252+00, Project No. 0672-6R6523-93, EI Sobrante area Specific Requests or Recommendations &Bic—kground&Justification I. Recommended Action: APPROVE Contract Change Order No. 3 in the approximate amount of $5,000.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Kulchin Condon & Associates, for work performed. II. Financial Impact: The project contingency funds are sufficient to cover this change order cost. The project is funded by Federal Aid Storm Damage Repair Funds (65.8%) and County Road Funds (34.2%). III. Reasons for Recommendations and Background: The cumulative total of Change Order Nos. 1 and 2 exceeds $25,000.00 requiring Board approval. IV. Consequences of Negative Action: Without approval, the contractor will. not receive payment for work required. Continued on attachment: _yes SIGNATURE.
