SUBJECT: Approval of Contract Amendment Agreement #22-580-1 with Edwin C.
McKetney
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-580-1 with Edwin C. McKetney, effective
December 1, 1996, to amend Contract #22-580, to increase the payment
limit by $37,400 from $25, 000 to a new Contract Payment Limit of
$62,400, and extend the term through June 30, 1997.
II. FINANCIAL IMPACT:
This Contract is funded 100% by State funds. No County match is
required.
III. REASONS FOR RECOMMENDATIONSIBACKGROUND:
In July 1996, the County Administrator approved and the Purchasing
Services Manager executed Short Form Service Contract #22-580 with
Edwin C. McKetney for the period from July 1, 1996 through December
31, 1996, for evaluation services for the Tobacco Prevention,
Healthy Neighborhoods, Breast Cancer Partnership and Partners in
Health Projects.
Approval of Contract Amendment Agreement #22-580-1 will allow the
Contractor to provide additional services through June 30, 1997.
