SUBJECT: Approval of Standard Contract #25-510-2 with Juliette Kelley
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Mary Foran) , to execute on behalf of the County, Standard Contract
#25-510-2 with Juliette Kelley, for the period from January 1, 1997
through December 31, 1997, with a payment limit of $53 ,700, for
consultation and technical assistance to the Department with regard
to, implementing programs within the Office of the Director—
II.
II. FINANCIAL IMPACT:
Funding for this Contract is included in the Department's Enterprise
I budget.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On December 19, 1995, the Board of Supervisors approved Standard
Contract #25-510 with Juliette Kelley for the period from December
18, 1995 through December 17, 1996 for provision of services for the
Department' s Office for Service Integration. Subsequent Contract
Amendment Agreement #25-510-1 extended Contractor's services through
December 31, 1996.
Approval of this Contract #25-510-2 will allow the Contractor to
continue providing services to the Department through December 31,
1997.
