Planned Parenthood: Shasta Diablo
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #22-354-6 with Planned Parenthood: Shasta-Diablo,
effective October 1, 1995, to amend Standard Contract #22-354-5, to
increase the Contract Payment Limit by $1,900, from $57, 800 to a new
total Payment Limit of $59, 700.
II. FINANCIAL IMPACT:
This contract is funded 100% by the State Department of Health
Services, Office of AIDS. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On June 20, 1995, the Board of Supervisors approved Standard Contract
#22-354-5 with Planned Parenthood: Shasta-Diablo, for the period from
July 1, 1995 through June 30, 1996, for AIDS education and prevention
services to women at increased risk of HIV infection.
The Department has received an augmentation award from the State
Office of AIDS for HIV education and prevention programs during FY
1995-96.
Approval of Contract Amendment Agreement #22-354-6 will allow the
Contractor to provide additional services through June 30, 1996.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's willingness to provide an
increased level of services under the Contract identified herein,
County agrees to increase the Contract Payment Limit. County and
Contractor agree therefore to amend said Contract as set forth
below while all other parts of the Contract remain unchanged and in
full force and effect.
1. Increased Payment Limit. The payment limit set forth in
Standard Contract #22-354-5, Paragraph 4 (Payment Limit) is hereby
increased by $1, 900, from $57 , 800 to a new total Contract Payment
Limit of $59,700.
2. Modification of Payment Provisions. The Payment Provisions
set forth in Paragraph 1 (Payment Amounts) , of Standard Contract
#22-354-5 is hereby deleted and replaced with a new subparagraph d,
to read as follows:
" [%] d. $ 4,816 monthly for services performed during the
months of July 1995 through September 1995, payable
upon demand, upon approval of the Health Services
Director, or his designee, of Contractor's monthly
written data collection and expenditure report, as
specified in the Work Plan, which is on file in the
County's AIDS Program Office, 597 Center Avenue,
Suite 200, Martinez, and which is incorporated
herein by reference; and
$ 5, 027 monthly for services performed during the
months of October 1995 through May 1996, payable
upon demand, upon approval of the Health Services
Director, or his designee, of Contractor's monthly
written data collection and expenditure reports, as
specified in the Work Plan; and
$ 5,036 for the month of June 1996; provided that
final contract payment will be withheld from
Contractor pending receipt of a properly documented
final demand and receipt by the Health Services
Director, or his designee, of Contractor's final
activity report specified in Paragraph 3 . (Progress
Reports) of the Service Plan. "
3. Modification of Work Plan. The Work Plan described in Service
Plan Paragraph 2 . (Service Activities) of Standard Contract #22-
353-5, shall be further modified to specify the additional services
which the Contractor has agreed to provide pursuant to this
Amendment.
