RECOMMENDED ACTION: Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D. ) to execute on behalf of the County, Contract Amendment Agreement #22-267-14 with AIDS Project of Contra Costa, effective October 1, 1995, to amend Standard Contract #22-267-13 (effective July 1, 1995 through June 30, 1996) , to increase the Contract Payment Limit by $18,410, from $79,500 to a new total Payment Limit of $97 ,910. II. FINANCIAL IMPACT: This contract is included in the Health Services Department's 1994-95 Fiscal Year Budget, and the funding source is the State Department of Health Services, Office of AIDS. $4, 000 in County funds are required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On June 6, 1995, the Board of Supervisors approved Standard Contract #22-267-13 with the AIDS Project of Contra Costa for Fiscal Year 1995- 96 for AIDS education and prevention services to men at increased risk of HIV infection. Subsequently, the Program received $18,410 in additional funding from the State and County. Approval of Contract Amendment Agreement #22-267-14 will allow the Contractor to provide additional outreach services through June 30, 1996. County and Contractor agree therefore to amend said Contract as set forth below while all other parts of the Contract remain unchanged and in full force and effect. 1. Increased Payment Limit. The payment limit set forth in Standard Contract # 22-267-13 Paragraph 4 (Payment Limit) is hereby increased by $18 , 410, from $79, 500 to a new total Contract Payment Limit of $97,910. 2 . Modification of Payment Provisions. The Payment Provisions set forth in Standard Contract #22-267-13 , Paragraph 1, subparagraph d (Payment Amounts) , is hereby deleted and replaced with a new subparagraph d, to read as follows: " [X] d. $ 6, 625 monthly for services performed during the months from July 1995 through September 1995, payable upon demand, upon approval of the Health Services Director, or his designee, of Contractor's monthly written data collection and expenditure report, as specified in the Work Plan, which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite 200, Martinez, and which is incorporated herein by reference; and $ 8, 670 monthly for services performed during the months of October 1995 through May 1996, payable upon demand, upon approval of the Health Services Director, or his designee, of Contractor's monthly written data collection and expenditure reports, as specified in the Work Plan, which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite 200, Martinez, and which is incorporated herein by reference; and $ 8, 670 for the month of June 1996; provided that final contract payment will be withheld from Contractor pending receipt of a properly documented final demand and receipt by the Health Services Director, or his designee, of Contractor's final activity report specified in Paragraph 3 . (Progress Reports) of the Service Plan. " 3 . Modification of Work Plan. The Work Plan described in Standard Contract 22-267-13, Service Plan Paragraph 2 (Service Activities) shall be modified to specify the additional services which the Contractor has agreed to provide pursuant to this Amendment.
