MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: November 14, 1995 SUBJECT: Approval of Contract Change Orders Nos. 11, 12, and 13 for the Appian Way Triangle Realignment, Project No. 0662-6R4056-88, EI Sobrante Area SpecificRequest(s)orRecommendation(s) Background&Justification 1. Recommended Action: APPROVE Contract Change Orders Nos. 11, 12, and 13, in the approximate amounts of$3,000.00 $7,500.00 and $3,500.00, respectively, and AUTHORIZE the Public Works Director to execute them in order to compensate the contractor, Joe Carone, Inc. for additional driveway and sidewalk work. II. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is State and Federal funds (60%), EI Sobrante A.O.B. (30%) and West County Wastewater District (10%). III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 13 exceeds $25,000.00, requiring Board approval. Due to special needs of some adjoining businesses, additional driveway widths and sidewalk replacements were constructed. IV. Consequences of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
