SUBJECT: Approve Standard Agreement (Amendment) #29-202-58 with the State
Department of Health Services for Family Planning Services
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County,
Standard Agreement (Amendment) #29-202-58 (State #95-20156-A1)
effective January 1, 1995 with the State Department of Health
Services to amend Standard Agreement #29-202-56 (effective July 1,
1994 through June 30, 1997) for the County' s Family Planning
Services. This amendment increases the. payment limit for FY 94-95
by $9,514 from a payment limit of $130, 486 to a new fiscal year
contract total of $140, 000.
II. FINANCIAL IMPACT:
Approval of this amendment will result in $9, 514 of 'increased State
funding for FY 1994-95 for the County' s Family Planning Services,
and increase the maximum reimbursable amount of State funding for
the three year term of the agreement to a new total of $400, 972 .
No additional County funding is required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For many years the County has provided family planning services to
citizens of child-bearing age who would otherwise be unable to
obtain these services. Family planning services funded through
this contract are for persons who request such services through
County facilities. The County must bill any other possible payment
source, such as Medi-Cal, before seeking reimbursement from the
State for services provided. This contract is a method for
reimbursement rather than a straight reimbursement contract.
Three sealed and certified copies of this Board Order should be
returned to the Contracts and Grants Unit for submission to the
State Department of Health Services.

A. Paragraph 2.A.(3) is hereby amended to read as follows:
"(3) EXHIBIT C-1, Payment Provisons, 7-1-94 to 6-30-95, Year 1
Upon the effective date of this amendment, all references to Exhibit C for fiscal year
1994/95 in the body of this agreement, and in any exhibits thereto, shall hereinafter be
deemed to read Exhibit C-1 for fiscal year 1994/95."
B. Paragraph 3.A is hereby amended to read as follows:
"A. The maximum amount payable under this agreement for services rendered during fiscal
year 1994-95 (July 1, 1994 - June 30, 1995) shall not exceed $140,000."

EXHIBIT C-1
PAYMENT PROVISIONS
OFFICE OF FAMILY PLANNING
JULY 1, 1994 - JUNE 30, 1995
YEAR 1
A. The Contractor shall have general spending authority at rates not to exceed those published in the
Guidebook for Family Planning Services, Part 3, entitled "Schedule of Maximum Allowances".
These payment provisions shall further be governed by the appropriate "Standards" in the
Guidebook, Division A.
B. The maximum amount payable under Exhibit C-1, shall be allocated as follows:
1. The Contractor shall have spending authority for providing contraceptive and pregnancy
testing services. The maximum amount shall not exceed $140,000.
There are no further subdivisions of this paragraph B.
