SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Standard Agreement (Amendment) #28-538-2 (State
#93-18517, 01) with the State Department of Health Services, to
increase the payment limit by $104, 800 and to extend the term of the
agreement through September 30, 1995.
II. FINANCIAL IMPACT:
This Agreement will result in an additional $104 ,800 in funding from
the State, from $107, 800 to a new total of $212, 600, for the second
year of a three year project for the County' s Pittsburg Active Living
Project. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Pittsburg Active Living Project is' a unique collaboration between
the County's Health Services Department, the City of Pittsburg's
Leisure Services Department and Healthy Cities Project, Los Medanos
Community College, medical providers, and an extensive list of
community based organizations. The goal of this Project is to reduce
risk factors for circulatory system disorders, cancer and diabetes
among the City of Pittsburg' s Latino, African-American, and
Asian/Pacific Islands adult residents.
On December 3 , 1993, the Board of Supervisors approved Standard
Agreement #28-538-1 with the State Department of Health Services, for
the period from 'October 1, 1993 through September 30, 1994 . Approval
of Standard Agreement (Amendment) #28-538-2 will extend the project
through September 30, 1995 and increase the funding by $104, 800.
Seven signed copies of the Agreement and three certified and sealed
copies of this Board Order should be returned to the Contracts and
Grants Unit.

C. Paragraph 4 is amended to read as follows:
4. The Contractor shall submit four progress reports in the format prescribed by the
State. The progress reports shall be submitted to the State by the 20th of the month
following the end of the second and sixth quarter period and within 30 days
following the end of the fourth and eighth quarter period. if said reports are not
received by the due dates, the'State may withhold 10 percent of the contract or 50
percent of a pending invoice, whichever is larger, but not to exceed $3,000.00
until receipt of a report satisfactory to the State.
D. Paragraph 7 is amended to read as follows:
7. The total amount payable by the State'to the Contractor under this contract shall
not exceed $212,600.
E. Paragraph 8 is amended to read as. follows:
8. The attached Exhibit B-1, entitled "Revised Budget", consisting of one (1) page is
incorporated herein and made a part hereof by this reference.
2. Upon the effective date of this agreement, all references to the Exhibit C, Project Workplan in
the body of this agreement and in all exhibits thereto shall be deemed to read Exhibit C-1, Revised
Project Workplan.
3. Upon the effective date of this agreement all references ophe term of the contract in the.body of
this agreement and in all references thereto, shall be deemed to read "October 1, 1993 through
September 30, 1995.
4. Upon the effective date of this agreement all references to Exhibit B, Budget, in the body of this
agreement and in all exhibits thereto shall be deemed to read Exhibit B-1, Revised Budget.
5. All other terms and provisions of this contract remain in full force and effect.

Goal Statement: The Pittsburg Active Living Project will create a community environment that
encourages and supports regular physical activity among the City of Pittsburg's Latino, African-
American, and Asian/Pacific Islander adult (aged 18 and older) residents.
Related National Year 2000 Objectives:
1.7 overweight persons adopt sound dietary and physical activity practices
1.12 50% of primary providers routinely assess and counsel patients
2.6 increase consumption of complex carbohydrates and fiber
2.21 increase proportion of primary providers who provide nutrition assessment, counseling, and .

Measurable Objective#1: By October 31, 1996, increase the proportion of Pittsburg adults to at least

concerning barriers to participation in physical activities experienced by Pittsburg
residents and to develop and implement strategies to improve the safety of parks and

population who engage in vigorous physical activity that promotes the development and maintenance
of cardio-respiratory fitness three or more days per week for at least 20 minutes per occasion.

2. Maintain community outreach efforts regarding the promotion of health and
exercise in Pittsburg's multi-ethnic community (level 2). 1-4
a. Community Health Advocates will collect information from community
agencies, organizations, religious institutions and businesses regarding
print media sources, physical activity programs or lack of physical
activity programs and interest in physical activity programs (Level 2). 1-4
b. Community Health Advocates will make presentations/conduct workshops
at conferences, religious institutions, community agency meetings and
other events to promote walking clubs and other existing physical
activities in the community (Level 2). 1-4
C. Community Health Advocates will distribute exercise promotion material
at various locations and make contact with Pittsburg's multi-ethnic
residents to promote the benefits of exercise, provide referrals to
community physical activities and promote awareness about the
Pittsburg ALP. project (Level 2). 1-4
d. A voice mail/message center will be established to provide information
on community physical activity events/resources as well as serve as a
method for contacting individuals interested in physical fitness and
health (Level 2). 2-3
3. Pittsburg ALP will participate in community events and meetings and will co-sponsor
at least one ethnic-appropriate community event to promote community-wide health and
physical fitness (Level 2). 1-3.
a. Community Health Advocates will enlist religious institutions (e.g.
Catholic and Baptist churches) to hold annual sermons linked with church
activity focusing on health/physical activity (Level 5). 1-4
b. Collaborate with community event coordinators to incorporate Pittsburg ALP
health and exercise messages as well as a physical activity component into
events such as Juneteenth, Community Relations Day and Community
Health Fairs (Level 4). 1-4

4. Pittsburg ALP will work with City of Pittsburg staff in developing "ON THE MOVE"
mileage markers in at least two city parks/recreational sites (Level 6). 1-3

ensure that mileage markers are created in at least two renovated/newly
developed parks and maintain the mileage markers as needed (Level 6). 1-3
b. Determine park sites/recreational locations for the Pittsburg ALP mileage

5. Develop ALP Walking and.Activity Clubs in at least 2-3 different areas/parks

method for conducting outreach and enrolling at least 5 low income adults in
at least one physical activity class during each of the three seasonal programs

b. Pittsburg ALP will recruit physical activity instructors who will teach
multi-ethnic dance such as African-Haitian, banda, salsa and folk dancing
through Pittsburg Leisure Services. Pittsburg ALP will sponsor the first
month of classes and subsequent classes would continue contingent upon interest

conducted in the community

the purpose of obtaining feedback regarding the implementation of PACE exercise
and counseling materials.
*Phase III: for a three month period conduct individualized training for implementation
of PACE exercise assessment and counseling maternal, community exercise referral
system and general exercise recommendations and background information.
*Phase IV: one month, six months and twelve months after PACE assessment and*
counseling component implementation,perform chart review and conduct patient survey
to determine changes in the number of,patients counseled pre and post intervention.
