Armstrong in the amount of $30,000 for consulting work involved in providing neighborhood service integration activities through December 31, 1995. BACKGROUND/REASON(S) FOR RECOMMENDATION(S): During the last six months, as part of the strategy of implementing the County mission of improving the health and well being of children and families through the redesigning of public services as a community-based and interagency model, Dr. Armstrong has provided significant assistance to various county departments. Contact John Gregory,646-4089 ATTESTED PHIL BATCIrELOR,CLERK OF THE BOARD OF SUPERVISORS AND COUNTY ADMINISTRATOR co: CAO Crown Zellerbach BY &4.4y&4.)DEPUTY y i . s`s - Approval of Contra Costa County as one of five AB 1741 counties to begin work with the state to obtain waivers to support service integration for children and families; • Development and implementation of an approved comprehensive neighborhood plan for after school recreation, education and community service for the North Richmond Community; • Introduction of the first small business development class in North Richmond with seven graduates submitting business plans for no interest loans in 1995; • Approval of a MAC proposal for supporting families of North Richmond inmates at California Department of corrections - Solano and providing training and jobs for inmates returning to North Richmond community; • Preparation and submission of MAC annual report and request for continued funding to funding partners Office of Child Abuse Prevention, Zellerbach Family Fund and Van Loben Sels Foundation; • Establishment of a GED class at Los Deltas Housing Project, funded by the MAC and the West Contra Costa Unified School District Adult Education Program. Now, through a generous contribution of $30,000 from the Zellerbach Family Fund in San Francisco, Dr. Armstrong will be able to continue and implement four assignments which further the service integration process. These assignments will be: 1) Work on the County's neighborhood services integration waiver proposal program; 2) Provide staff support to the North Richmond Municipal Advisory council for implementation of its community action plan; 3) Consult with outside agencies regarding our County's efforts to implement neighborhood service integration; 4) Provide staff support to the Fiscal Sub-Committee on a financial strategy for supporting community-based services. The above activities of this consultant should assist the County in its efforts to direct services more effectively and efficiently to identified target populations. 2 SHORT FORM SBRVICS CONTRACT Contra Costa County Number Standard Form 1/86 Fund/Org / Account # Other / 1. Contract Identification. Department: County Administration Subject: Waiver Study and Negotiations and Assistance to North Richmond MAC Program Manager 2. Parties. The County of Contra Costa, California (County), for its Department named above, and the following named Contractor mutually agree and promise as follows: Contractor: Katherine L. Armstrong Capacity: Consultant on Service Integration Taxpayer ID$ Address: 5 Admiral Drive, #219 Emeryville, CA 94608 3. Term. The effective date of this Contract is January 2, 1995 and it terminates December 31, 1995 unless sooner terminated as provided herein. 4. Termination. This Contract may be terminated by the County, at its sole discretion, upon five-day advance written notice thereof to the Contractor, or cancelled immediately by written mutual consent. 5. Payment Limit. County's total payments to Contractor under this contract shall not exceed $30,000 6. Count 's Obli ations. In consideration of Contractor's provision of services as described below, and su'1 '.2, to the payment limit expressed herein, County shall pay Contractor, upon Contractor's submission of a.properly documented demand for payment (County Demand Form D-15) which shall be submitted not later than 45 days from the end of the month in which the contract services were rendered, and upon approval of such demand by the head of the County Department for which this Contract is made or his designee, as follows: [Check one alternative only] (X) hour; or [X] a. Contractor shall provide the following described services: See Attached Scope of Work B. Compliance with Law. Contractor shall be subject to and comply with all Federal, State and local laws and regulations applicable with respect to its performance under this Contract, including but not limited to, licensing, employment and purchasing practices; and wages, hours and conditions of employment, including nondiscrimination. 9. Nondiscriminator Services. Contractor agrees that all goods and services under this Contract shall be availa le to al quali ied persons regardless of age, sex, race, religion, color, national origin, or ethnic background, or handicap, and that none shall be used, in whole or in part, for religious worship or instruction. This Contract is by and between two independent contractors and is not inten .gel_ ind shall not b7a construed to create the relationship between the parties of agent, servant, employee, partnership, joint venture, or association. 11. D�' te�s. Disagreements between the County and Contractor concerning the meaning, requirements, or performance of this Contract shall be subject to final determination in writing by the head of the County Department for which this Contract is made or his designee or in accordance with the applicable procedures (if any) required by the State or Federal Government. 12. Access to Books and Records of Contractor, Subcontractor. Pursuant to Section 1861(v) (1) of the Social Security Act, and any regulations promulgated t ereun er, Contractor shall, upon written request and until the expiration of four years after the furnishing of services pursuant to this Contract, make available to the Secretary of Health and Human Services, the Comptroller General, the County, or any of their duly authorized representatives, this Contract and books, documents, and records of Contractor that are necessary to certify the nature and extent of all costs and charges hereunder. Further, if Contractor carries out any of the duties of this Contract through a subcontract with a value or cost of $10,000 or more over a twelve-month period, such subcontract shall contain a clause to the effect that upon written request and until the expiration of four years after the furnishing of services pursuant to such subcontract, the subcontractor shall make available, to the Secretary, the Comptroller General, the County, or any of their duly authorized representatives, the subcontract and books, documents, and records of the subcontractor that are necessary to verify the nature and extent of all costs and charges hereunder. This special condition is in addition to any and all other terms regarding the maintenance or retention of records under this Contract and is binding on the heirs, successors, assigns and representatives of Contractor. ' 13. Reporting Requirements. Pursuant to Government Code §7550, Contractor shall include in all documents or written reports completed and submitted to County in accordance with this Contract, a separate section listing the numbers and dollar amounts of all contracts and subcontracts relating to the preparation of each such document or written report. This section shall apply only if the payment limit under this Contract exceeds $5,000. 14. Indemnification. The Contractor shall defend, indemnify, save, and hold harmless the County and its officers an employees from any and all claims, costs and liability for any damages, sickness, death, or injury to person(s) or property, including without limitation all consequential damages, from any cause whatsoever arising directly or indirectly from or connected with the operations or services of the Contractor or its agents, servants, employees or subcontractors hereunder, save and except claims or litigation arising through the sole negligence or sole willful misconduct of the County or its officers or employees. Contractor will reimburse the County for any expenditures, including reasonable attorneys fees, the County may make by reason of the matters that are the subject of this indemnification, and if requested by the County will defend any claims or litigation to which this indemnification provision applies at the sole cost and expense of the Contractor. 15. Le al Authorit . This Contract is entered into under and subject to the following legal authorities: Cali ornia Government Code Sections 26227 and 31000. 16. Signatures. Designee Individual Contractor Approved: County Administrator Designate official capacity) By Designee Recommended by Department By Designee hs/c:\wp51\shrtfrm.shl (Form approved by County Counsel) SCOPE OF WORK During 1995, Katherine Armstrong will be involved with four major assignments: Contra Costa Waiver Proposal, MAC support, San Francisco Social Services and the Center for Integrated Services activities. Approximately $10,000, about 14 hours a month, for 12 months, will available to Contra Costa County to: • Assist in its efforts to obtain waivers from the state and federal government needed to continue efforts in implementing community based, family centered, integrated services in the nine identified neighborhoods; • Provide consultation to the North Richmond Municipal Advisory Council (MAC) Program Manager, D'Andre Wells, and John Gregory, Deputy County Administrator, in their efforts to oversee and implement the MAC's Community Action Plan; • $15,000 or about 21 hours a month, for 12 months will be available to San Francisco County for: • Consulting with San Francisco Department of Social Services regarding the lessons and experiences gained through Contra Costa County's efforts to implement neighborhood service integration; • _ Staff support to the Fiscal Sub Committee, which meets each month. In this role, will facilitate the Fiscal Committee's meetings, help with data collection, staff coordination, and analyses needed to develop an inter- agency financial strategy for supporting community based services to children and families; Specifically, work with the fiscal sub-committee to: • Assess how the County can maximize the use of state and federal funding streams; • Identify current available funding streams and determine to what degree these funds could be pooled into an inter-agency unified budget for funding children's services; • Review approaches used by other counties or states to combine funds to support consolidated, re-structured,services to communities and families. • Develop a set of recommendations for short and long term fiscal strategies for maximizing existing state and federal dollars and for pooling, re-directing, or consolidating existing funding sources to increase financial support for inter-agency integrated services. and, $5000 or about 7 hours a month will be allocated to assist the Center for Integrated Services to: • Help coordinate presentations to the greater community on Contra Costa County's efforts in North Richmond and Bay Point; • Consult with Cathy Minicucci and Associates in the application of the evaluation of the Sacramento Del Paso Neighborhood Pilot to Contra Costa County. Dr. Armstrong will report to the County Administrator or his delegate regarding the performance of the contract. Katherine Armstrong will submit monthly invoices by the first of each month stating the number of hours work and services provided during the month.
