No. 95/ 53 UAPD 1993-1995 MOU ) 1 . On March 1 , 1994, the Board adopted Resolution 94/118 which approved a Memorandum of Understanding between the County and the Union of American Physicians and Dentists (UAPD) containing various terms and conditions of employment for the contract period October 1 , 1993 through December 31 , 1995 2. Section 5.1 (G) Salary Structure of the MOU contains provision for award of additional pay on or after January 1 , 1995 in the amount of one percent (1 Yo) to employees in the Unit based upon enhanced productivity criteria which was to be developed by a joint labor/management committee. 3. The Director of Health Services has accepted a final recommendation from the labor/management committee that the incentive criteria be in the form of an on-going patient satisfaction survey mechanism which assesses provider service levels and allows corrections for enhanced effectiveness and competitiveness and has recommended that Section 5.1 (G) be implemented effective February 1 , 1995 in consideration thereof; NOW THEREFORE: The Contra Costa County Board of Supervisors in its capacity as governing Board of the County of Contra Costa, and all districts of which it is the ex-officio governing Board, RESOLVES THAT effective February 1 , 1995, all classifications represented in the Physicians and Dentists Unit be adjusted upward on the salary schedule by ten (10) levels. 1 hereby certify that this is a true and correct copy of an action taken and entered on the minutes of the Board of Supe isTs on the date shown. 't COUri'� TO: Leslie Knight Director of Personnel Att: Rich Heylne La r R.-': j ' ations Manager FROM: Mark Finucan Director of Health Services DATE: January 3, 1995 SUBJECT: UAPD Productivity Award This is to advise you that, pursuant to section 5. 1G. of the Memorandum of Understanding between the County and the Union of American Physicians and Dentists (UAPD) , the labor-management committee has met and conferred in good faith and has fulfilled its responsibility to develop a process. to measure enhanced "productivity". During the course of those discussions, it was clear that both the Union and the Department feel strongly that any system that pits one individual against another or measures individual merit, is doomed to fail. The old "incentive pay" system already proved that. Similarly, measuring "productivity" in the traditional sense can have a disastrous negative consequence on our system. For example, a physician who increases the number of patients seen on a given day, may do a more cursory exam, resulting in the patient returning more frequently, or to a more expensive setting, such as the Emergency Department or surgery. There was unanimous agreement by the committee that,the goal of the project must be to enhance the system's competitiveness. Since the physicians and dentists cannot control the efficiency and effectiveness of non-provider functions, the decision was made to do a "user" survey, measuring satisfaction with provider services only. In consultation with the Health Plan Director, it was determined that patient satisfaction with the provider is a significant determinant of competitiveness. We expect to do a test run early in 1995 and a full run in June or July, in order to establish a base line for what may become semi-annual surveys. When the MoU was executed, it was the intent of both parties to have a system in place that would result in awarding the 1% by the beginning of 1995. Although the committee worked in good faith over several months, we underestimated the difficulty we would have in defining the goals and guiding principles leading to the final plan. Since we have met the spirit of section 5. 1G and will be moving ahead with the plan, I am recommending that the 1% be awarded across the board, effective 2/1/95. The physicians and dentists have clearly demonstrated a commitment to the future success of our health plan and services, and should be given appropriate recognition. As a significant side benefit, the survey will allow us to use the patients' perceptions as a basis for allocation of additional limited resources to further enhance competitiveness.
