RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute, on behalf of the County, Cooperative Agreement Amendment #28-562-2 (Federal Grant #58-3194-4-051) with the U.S. Department of Agriculture (USDA) , Food and Consumer Service (formerly: Food and Nutrition Service) , to increase the contract payment limit by $109,997, from $104, 330 to a new total of $214, 327, and to extend the term of the Agreement from September 29, 1995 through September 29, 1996, for the Cooperative Nutrition Education/Food Wise Project. II. FINANCIAL IMPACT: This amendment increases the USDA's funding for this project by $109,997, for a new total of $214, 327, and continues the Cooperative Nutrition Education/Food Wise Project through September 29, 1996. No County match is required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: The goal of the Cooperative Nutrition Education/Food Wise Project is to improve nutritional well-being and reduce risk for chronic disease among food stamp household members, school meal participants, and children participating in the Child Care Council 's Child Care Food Program, through the establishment and maintenance of a cooperative nutrition education service. Approval of this Cooperative Agreement Amendment #28-562-2 will allow the Department's Prevention Program and the Food and Nutrition Policy Consortium to continue to provide nutrition education to Contra Costa County food assistance recipients through classes and workshops, as well as through existing community channels, such as schools and food distribution sites, through September 29, 1996. Four signed copies of the amendment and three certified/sealed copies of the Board Order should be returned to the Contracts and Grants Unit.
