RECOMMENDED ACTION: Approve and authorize the Health Services Director to execute on behalf of the County, Standard Agreement (Amendment) #29-609-43 (State #93-18608 A-02) with the State Department of Health Services, effective August 1, 1995, to amend Standard Agreement #29-609-41 (as amended by Standard Agreement [Amendment] #29-609-42) , for prepaid health services for Medi-Cal beneficiaries. This amendment increases the FY 1995-96 payment limit by $30,467,796 from $18,232,204, for a new total of $48,700,000, and extends the term of the agreement through June 30, 1998. II. FINANCIAL IMPACT: Approval of this amendment will increase the State's funding encumbrance for FY 1995-96 by $30,467,796, for a new total of $48,700,000. However, the net effect of this increase on Health Plan revenues is dependent upon enrollment levels. III. REASONS FOR RECOMMENDATIONSIBACKGROUND: On January 11, 1994, the Board approved Standard Agreement #29-609-41 (as amended by Standard Agreement [Amendment] #29-609-42) with the State Department of Health Services, for prepaid health services for Medi-Cal beneficiaries. Standard Agreement (Amendment) #29-609-43 increases the enrollment limit from 15,000 to 40,000 Medi-Cal members, reflects the associated revenue increases, and extends the term of the agreement through June 30, 1998. Three .sealed and certified copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the State Department of Health Services. Contra Costa County Board of Supervisor 651 Pine Street #/106 Martinez, CA 94553 Dear Ms. Bishop: The Alameda-Contra Costa Medical Association (ACCMA) wants to explore how the Claremont Medical Group, Inc. (CMG) can participate in a strategic alliance with Contra Costa Health Plan (CCHP) through a single IPA managed medical care contract to serve the County's Medi-Cal population under the Local Initiative. We believe a joint venture between ACCMA and Contra Costa County sponsored organizations such as CMG and CCHP will offer many advantages to the community. The ACCMA is a professional organization with board representation throughout CCHP's service area. Community physicians have provided Medi-Cal patients medical services since the programs inception, and have shared the community responsibility for providing care to the underserved. The CCHP/CMG arrangement has the potential of being an important partner with the County that ensures health care services to the vulnerable portion of the service population. The volume of eligible, managed care members who choose the Local Initiative through one IPA may result in cost effective delivery of care that fits within the costs' contraints of the proposed State capitation rate. The Claremont Medical Group, Inc. wants to work closely with the Contra Costa. Health Plan to ensure the successful transition of the Local Initiative to serve the Medi-Cal managed care patients.
