Eya SPECIFIC REQUEST(S) OR RECOMMENDATION(S) ac BACKGROUND AND JUSTIFICATION ' I. RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee (Chuck Deutschman) , to execute on behalf of the County, Contract Amendment Agreement #24-769-3 with Eileen T. Eya, effective November 1, 1995, to amend Standard Contract #24-769-1 (as amended by Contract Amendment Agreement #24-769-2) to modify the Payment Provisions to transfer funds from one expense category to another, with no increase in the Contract Payment limit. II. FINANCIAL IMPACT: None. There is no change in the Contract Payment Limit, and this Contract is funded 100% by Federal (HIV Set-Aside) Block Grant funds. No County funds are required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On October 4, 1994, the Board of Supervisors approved Novation Contract #24-769-1 (as amended by Contract Amendment Agreement #24- 769-2) with Eileen T. Eya for consultation and technical assistance to the Department with regard to implementing an HIV/Communicable Disease. Testing and Education Program. Contractor is providing services under the Automatic Extension of Contract #24-769-1. This Contract Amendment Agreement decreases the maximum reimbursement amount for seminar/conference expenses and increases the maximum amount the county will reimburse Contractor for mileage and miscellaneous expenses, to allow the contractor to continue to travel on behalf of the County through December 31, 1995. County while providing services under Contract #24-769-1 identified herein, County agrees to increase the amount it will reimburse Contractor for mileage and needed supplies. Increase in Mileage Expense Reimbursement. The reimbursement amount set forth in Special Conditions Paragraph 3 . (Automatic Contract Extension) , subparagraph b. (2) . , for needed supplies and for daily use of a private automobile following arrival at the work site at the rate of 29 cents per mile, is hereby increased by $650, from 750 to a new total amount not to exceed $1,400. 2 . Decrease in seminars and Conference Expense Reimbursement. The reimbursement amount set forth in Special Conditions Paragraph 3 . (Automatic Contract Extension) , subparagraph b. (3) . , for allowable expenses actually incurred while attending training seminars and conferences on behalf of the County, is hereby decreased by $650, from 750 to a new total amount not to exceed 100.
