RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract Amendment Agreement #22-468-5 with Pittsburg Preschool Coordinating Council, effective October 1, 1995, to decrease the payment limit by $101483, from $36, 983 to a new Contract payment limit of $26, 500. This Contractor provides outreach and referral services for County's STD Mobile Clinic Project and. the TB Directly Observed Therapy Program. II. FINANCIAL IMPACT: This Contract is funded 100% by State funds. No County funds are required. The Department was recently notified that the State would reduce funding for the County' s STD Mobile Clinic Outreach Project. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On October 17, 1995, the Board of Supervisors approved Standard Contract #22-468-4 with Pittsburg Preschool Coordinating Council, Inc. , to provide outreach and referral services for County' s STD Mobile Clinic Project and the TB Directly Observed Therapy Program. Approval of Contract Amendment Agreement #22-468-5 will allow the Department to make the proper adjustments in the \Contrac,�t payment limit to reflect the recent reduction in State funds, for"-the STD Mobile Clinic Project. Payment Limit Decrease. The payment limit set forth in Contract Paragraph 4. (Payment Limit) is hereby decreased by $10,483, from 36 983 to a new total Payment Limit of $26 ,500. 2. Modification of Contract Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts) , subparagraph d. , is hereby deleted and replaced with a new subparagraph to read as follows: " [X] d. (1) $ 3,081.00 monthly, after receipt by the Department of a properly documented demand (D-15) and approval by the Health Services Director., or his designee (Communicable Disease Program Director) of Contractor's reports as set forth in Paragraph 3. (Reports) of the Service Plan, payable after the first day of the following months: July, 1995 through October, 1995; and (2) $ 2,963.00 monthly, after receipt by the Department of a properly documented demand (D-15) and approval by the Health Services Director, or his designee, of Contractor' s reports as set forth in Paragraph 3. (Reports) of the Service Plan, payable after the first day of the following months: November 1995 through December, 1995; and (3) $ 1,375.00 monthly, after receipt by the Department of a properly documented demand (D-15) and approval by the Health Services Director, or his designee, of Contractor's reports as set forth in Paragraph 3. (Reports) of the Service Plan, payable after the first day of the following months: January 1996 through May 1996; and (4) $ 1,375.00 payable after receipt by the Department of a properly documented final demand (D-15) and after approval by the Health Services Director, or his designee, of Contractor's final report as set forth in Paragraph 3. (Reports) of the Service Plan. " 3. Performance Reports . Service Plan Paragraph 3. (Reports) is hereby deleted and replaced with a new paragraph to read as follows: "Performance Reports. Contractor shall prepare and submit to County periodic performance progress reports in the time, form and manner required by County's Health Services Director, or his designee. No later than August 15, 1996, or 60 days following the termination of this Contract, whichever comes first, Contractor shall prepare and submit to County a Final Contract Performance Report, in the form and manner prescribed by County's Department Director, or his designee. " 4. Modification of Work Plan. The Work Plan described in Service Plan Paragraph 2.
