Goodman & Associates
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Lorna Bastian) , to execute on behalf of the County, Contract
Amendment Agreement #24-692-2 with Carol Goodman & Associates,
effective September 15, 1994, to amend Contract #24-692 (as extended
by Contract Extension Agreement #24-692-1) , to increase the contract
payment limit by $20, 165, from $19, 800 to a new total payment limit of
$39,965.
II. FINANCIAL IMPACT:
This Contract is funded by County/Realignment 100%.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In January, 1994, the County Administrator approved, and the County's
Purchasing Services Manager executed, Contract #24-692 effective
December 20, 1993 through June 30, 1994. (as extended by Contract
Extension Agreement #24-692-1) , with Carol Goodman & Associates, to
provide consultation and technical assistance to the Department with
regard to a managed care system for Medi-Cal mental health services.
Approval of Contract Amendment Agreement #24-692-2 will allow the
Contractor to provide additional services through June 30, 1995.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's willingness to provide additional units of
service under the Contract identified herein (as extended by Contract
Extension Agreement #24-692-1) , County agrees to increase the contract
payment limit. County and Contractor agree, therefore, to amend said
Contract as set forthl below while all other parts of the Contract remain
unchanged and in full force and effect.
1. Payment Limit Increase. The total contract payment limit set forth in
Paragraph 4 . (Payment!j Limit) of Contract Extension Agreement #24-692-1 is
hereby increased by $20, 165, from $19,800 to a new total Payment Limit of
$39,965.
2 . Modification of Payment Provisions. Paragraph 6. (County's
Obligations) , subparagraph a. , is hereby modified to read as follows:

NOT TO EXCEED a total of 399 service unit(s) . In addition, County will
reimburse Contractor an amount not to exceed $4, 055, as follows: i) for

advance by the Health Services Director or his designee; ii) for
miscellaneous expenses actually incurred in the provision of services
hereunder during the term of this Contract; and iii) at the rate of 29
cents per mile for the use of a private automobile required in the
provision of services hereunder. "
