MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: January 25, 1994 SUBJECT: Approval of Contract Change Orders Nos. 10, 12, 13, 14, and 15 for the A.D. 1992-1; Mrack Road project,' Danville area, Project No. 4500-6X5181-92 Specific Requests)or Recommendations)&Background&Justification I. Recommended Action: APPROVE Contract Change Orders Nos. 10, 12, 13, 14, and 15 in the approximate amounts of $3,800.00, $300.00, $5,900.00, $300.00, and $2,100.00, respectively, and AUTHORIZE the Public Works Director to execute them in order to compensate the contractor, Oliver de Silva, Inc., for work resulting from utility conflicts. II. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Assessment District 1992-1; Mrack Road. III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 15 exceeds$25,000.00, requiring Board approval. The sequence and scope of work was altered because of encountering buried utilities. These unforseen conditions resulted in additional force account work. IV. Consequences of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
