RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute on behalf of the County, Contract Amendment Agreement (Reformation) #24- 588-6, to reform Contract #24-588-4 (as cancelled by Contract Cancellation Agreement #24-588-6) with Family Stress Center, to remedy the mutual mistake of the County and Contractor and to accurately reflect the intent of the parties. II. FINANCIAL IMPACT: None. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On October 6, 1992 , Board of Supervisors approved Standard Contract #24-588-4 with Family Stress Center, for the period from September 1, 1992 through August 31, 1993, for the provision of substance abuse assessment and education coordination services for the County's Born Free Project. On July 13 , 1993, your Board approved Contract Cancellation Agreement #24-588-5, a mutual consent cancellation, to cancel Standard Contract #24-588-4, effective April 1, 1993 . Due to the mutual mistake of the parties, this Contract did not accurately reflect the intent of the parties which was that Family Stress Center continue providing services under the aforementioned contract until the close of the workday on April 15, 1993 . Approval of Contract Amendment Agreement (Reformation) #24-588-6 will reform the Contract to change the effective date of the mutual consent cancellation to April 15, 1993 and to provide for payment to the Contractor for services which it provided to the County during the month of April 1993 .
