RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee (Lorna Bastian) , to execute on behalf of the County, Contract Amendment Agreement #24-666-2 with Nancy Brewster, to extend Contract #24-666, as amended by Contract Amendment Agreement #24-666-1, from December 31, 1993 to April 30, 1994 , and to add $16, 000 to the automatic extension payment limit, from $27 , 324 to a new total of $43 , 324 . II . FINANCIAL IMPACT: This contract is funded by Federal SAMHSA Block Grant Funds. No County funds are required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: This Contractor has been providing consultation and technical assistance to the Department' s Mental Health Division for implementation of the Federal Substance Abuse and Mental Health Services Administration (SAMHSA) Formula Grant under a six-month automatic contract extension of Contract #24-666, as amended by Contract Amendment Agreement #24-666-1. Approval of Contract Amendment Agreement #24-666-2 will allow the Contractor to provide additional hours of service to facilitate the implementation of Coordinated Services for the Mental Health Division, through April 30, 1994 . Modification of Subject. The Subject specified in Paragraph 1. (Identification of Contract to be Amended) of Contract Amendment Agreement #24-666-1 is hereby modified to read as follows: "Subject: Consultation and technical assistance with regard to implementation of the Federal Substance Abuse and Mental Health Services Administration (SAMHSA) Formula Grant, and Coordinated Services for the County's Mental Health Division" 2. Modification of Automatic Contract Extension. Paragraph 3. (Automatic Contract Extension) of Contract Amendment Agreement #24-666-1 is hereby modified to read as'follows: 114. Automatic Contract Extension. Notwithstanding Paragraph 3. (Term) of this Contract, unless this Contract is terminated by either party pursuant to Paragraph 4. (Termination) prior to June 30, 1993, the term of this Contract shall be automatically extended from June 30, 1993 through April 30, 1994. During its extended term, this contract is nevertheless subject to all the terms and conditions applicable during its initial term, including but not limited to Contract Paragraph 4. (Termination) , except as to payment for services rendered during the extended term. The purpose of this automatic contract extension is to allow for continuation of services as specified in this Contract, to avoid interruption of payment to Contractor, and to allow County time in which to complete a novation or renewal contract for Contractor and County Board of Supervisors approval. As to any such automatic contract extension: a. The Contract Payment Limit, specified in Paragraph S. (Payment Limit) of this Contract, is increased by $43,324 (the automatic contract extension Payment Limit) and County's total payments to Contractor for said period shall not exceed this Payment Limit, subject, nevertheless, to the aforesaid novation or renewal contract. b. Subject to the Payment Limit specified above, County shall pay Contractor as follows: (1) $32.00 per hour, not to exceed 1332 hours of service for a total of $42 ,624; and (2) County will also reimburse Contractor an amount not to exceed 700 as follows: (a) for travel on behalf of the County, approved in advance by the County's Health Services Director or his designee; (b) for miscellaneous expenses actually incurred in the provision of services hereunder during the term of this Contract; and (c) at the rate of 28 cents per mile for the use of a private automobile required in the provision of services hereunder. lh� Y Initials: Contractor County Dept. 1 AMENDMENT SPECIFICATIONS Number 24-666-2 C. Contractor shall continue to provide services as set forth in Paragraph 7. (Contractor's Obligations) , subject to any amendments thereto. d. This six-month contract extension shall be subject to any further agreement (novation) which Contractor and County may enter into covering the provision of services during the contract period immediately following the contract period specified in Paragraph 3. (Term) . " 3. Modification of Contractor's Obligations. Paragraph 7. (Contractor's Obligations) Contract #24-666 is hereby modified to read as follows: 7. Contractor's Obligations. Contractor shall provide the following described services: Consultation and technical assistance with regard to implementation of the Federal SAMHSA Grant, including, but not limited to: ensuring that the Department receives full revenue reimbursement under the Grant; and training County-selected staff with regard to the Grant Project. In addition, Contractor shall provide consultation and technical assistance to facilitate the implementation of Coordinated Services for the Department's Mental Health Division.
