MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: February 15, 1994 SUBJECT: Approval of Contract Change Order No. 18 for the A.D. 1992-1 project, Project No. 4500-6X5181-92, Danville area Specific Request(s)or RecomnxwxJa i m(s)&Background&Justification I. Recommended Action: APPROVE Contract Change Order No. 18 in the approximate amount of $2,800.00, and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Oliver de Silva, Inc., for work resulting from utility conflicts. II. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Assessment District 1992-1. III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 18 exceeds$25,000.00, requiring Board approval. The sequence and scope of work was altered because of encountering buried utilities. These unforseen conditions resulted in extra work. IV. Consequences-of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
