SUBJECT: Fee-For-Service/Medi-Cal Acute Psychiatric Inpatient Hospital Services Transfer to the County
SPECIFIC REQUEST(S)OR RECOMMENDATIOM(S) &BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
1. Adopt Board Resolution No. 94/ 617: An agreement with the State to assume responsibility for Fee-For-
Service (FFS ) Medi-Cal acute psychiatric inpatient hospital services as of January 1, 1995.
2. Authorize the Health Services Director, Mark Finucane, or his designee (Lorna Bastian, Mental Health
Director) to negotiate and execute contracts for acute psychiatric inpatient services with East Bay Hospital, First
Hospital Vallejo and CPC Walnut Creek for the period January 1, 1995 through December 31, 1995.

FISCAL IMPACT:
The State allocation for acute psychiatric inpatient services for the period January 1, 1995 to June 30, 1995
is $804,135. This represents the projected cost of FFS acute psychiatric inpatient based on historical State
General Fund costs and anticipated growth in Med-Cal beneficiaries. Subsequent allocations will be for the
full fiscal year.
Under terms of the agreement with the State, as delineated in Board Resolution No. 94/617 , $804,135
constitutes full payment for FFS acute psychiatric inpatient services for the period January 1, 1995 through June
30, 1995. The County is at risk for costs in excess of this amount.
REASONS FOR RECOMMENDED ACTION AND BACKGROUND INFORMATION:
1. Chapter 633, Statutes of 1994, AB 757, authorized the transfer of state funding for Fee-For-Service/Medi-
Cal (FFS/MC) acute psychiatric inpatient hospital services from the Department of Health Services to the
Department of Mental Health (DMH). DMH will transfer these funds and the responsibility for authorization
and funding of Medi-Cal acute psychiatric inpatient hospital services to counties that choose to participate in
this program. This transfer will take place on January 1, 1995.
The State Department of Mental Health issued its Medi-Cal Managed Mental Health Care Plan on June 1,
1994. The State's Plan provides for statewide implementation of a single Managed Mental Health Care Plan
in each county. The implementation of the State's Plan , a capitated mental health system, will be
accomplished through a series of planned phases. The first phase of this Plan is the transfer to the counties
of the acute psychiatric inpatient FFS system and consolidating it with the existing county ental health system.

On July 12, 1995, your Board notified the State of its intent to function as the Local Mental Health Care Plan
and to assume the responsibility of inpatient consolidation. Board Resolution No. 94/ confirms the
County's agreement to assume responsibility for FFS acute psychiatric inpatient services.
2. To implement the Inpatient Consolidation program on January 1, 1995, the County must have contracts in
place with hospitals providing acute psychiatric services. The State's Plan requires that counties contract with
those hospitals which are designated "safety net providers" and certain hospitals designated as "traditional
providers". Accordingly, Contra Costa County is required to contract with East Bay Hospital (a safety net
provider) and First Hospital Vallejo (a traditional provider). The County has in the past contracted with CPC
Walnut Creek Hospital for the provision of acute psychiatric services for children and adolescents, as the
County does not admit children and adolescents to its psychiatric units at Merrithew Memorial Hospital (by
Board of Supervisors Policy). The Department proposes continuing to contract with this facility for these
services under inpatient consolidation.
The Health Services Department, Mental Health Division is currently in discussions with these providers for
provision of acute psychiatric services. To assure that contracts are in place by January 1, 1995, and because
of the limited number of Board of Supervisors meetings before January 1; we are asking for authority to
negotiate and execute, with County Counsel approval, contracts with these three facilities.
CONSEQUENCES OF NEGATIVE ACTION:
If the County does not assume responsibility for FFS Medi-Cal acute psychiatric inpatient services, the State
would contract with another governmental or non-governmental entity to do so. Under provisions of AB 757,
this entity would then assume responsibility for all Mental Health Medi-Cal services, including that which is
currently part of the County Mental Health System. This would result in a loss of those realignment dollars
that are used to match federal Medicaid dollars.

The Contra Costa County Board of Supervisors:
1. Agrees as of Jaunuary 1, 1995 to assume responsibility for Medi-Cal authorization and payment of the state
Medi-Cal match for all medically necessary acute psychiatric inpatient hospital services for Medi-Cal
beneficiaries from Contra Costa County.
2. Acknowledges that the allocation of state funds, $804,135 for the period January 1,1995 to June 30, 1995,
is payment in full from the State.
3. Authorizes the State to offset the County's realignment funds in accordance with state match requirements
for reimbursement of inpatient claims for Contra Costa County residents.
4. Assures that the County has a public planning process which involves traditional and safety net providers,
consumers and other stakeholders.
5. Agrees to submit additions to the Quality Management Plan by January 1, 1995, whch complies with state
requirements.
